摘要
目的比较结膜下注射曲安奈德和经球内注射曲安奈德治疗葡萄膜炎继发黄斑水肿的效果。方法选择我院2015年10月~2016年10月收治的葡萄膜炎继发黄斑水肿患者60例,共78眼,按随机数表法分为研究组和对照组各30例,各为39眼。研究组患者给予结膜下注射曲安奈德,对照组患者给予球内注射曲安奈德。比较两组患者的治疗效果,治疗前后黄斑中心凹视网膜厚度、最佳矫正视力变化以及患者治疗后不良反应及复发的发生情况。结果研究组的总有效率明显高于对照组,差异具有统计学意义(P<0.05);治疗前,两组患者的黄斑中心凹视网膜厚度以及最佳矫正视力比较,差异均无统计学意义(P>0.05),治疗后,研究组的黄斑中心凹视网膜厚度明显低于研究组,且最佳矫正视力高于对照组,差异比较具有统计学意义(P<0.05);治疗后研究组的眼压情况、结膜下出血情况、炎性反应和视网膜脱落等与对照组比较,差异无统计学意义(P>0.05),但研究组的复发率和总发生率明显低于对照组,差异均具有统计学意义(P<0.05)。结论结膜下注射曲安奈德较经球内注射曲安奈德对葡萄膜炎继发黄斑水肿的治疗效果良好,对提高患者视力,改善患者黄斑水肿情况有很大作用,且可以降低不良反应发生率和复发率,值得在临床广泛应用。
Objective To compare the effect of subconjunctival injection of cu ann ned and cu ann ned injection for macular edema secondary to uveitis. Methods 60 cases of macular edema secondary to uveitis（78 eyes） cured in our hospital from October 2015 to October 2016 were selected. According to the random number table method, they were divided into study group and control group with 30 cases（39 eyes） in each. Patients in study group were treated with subconjunctival injection of cu ann ned, and patients in control group were treated with intra injection cu ann ned. The therapeutic effect, the macular fovea thickness, the corrected visual acuity changes, and the adverse reactions and recurrence ~f the patients after treatment of the two groups were compared. Results The total effective rate of the study group was higher than that of the control group, the difference was statistically significant（P 〈 0.05）. Before treatment, there were no statistically significant differences in macular foveal thickness, retinal thickness, and corrected visual acuity between the two groups（P 〉 0.05）. After treatment, the naacular fovea thickness in the study group was significantly lower than that in the study group, and the best corrected visual acuity was higher than the control group, the difference was statistically significant（P 〈 0.05）. After treatment, the IOP, subconjunctival hemorrhage, inflammatory reaction and retinal detachment in the study group were not significantly different from those in the control group（P 〉 0.05）. Conclusion Subconjunctival injection of ca ann ned is more effective than cu ann ned in the treatment of macular edema secondary to uveitis. It plays an important role in improving the vision of patients and improving macular edema. And it can reduce the incidence of adverse reactions and recurrence rate, it is worthy of widely using in clinical.
