中重度急性呼吸窘迫综合征患者俯卧位通气的呼吸道管理被引量：21

摘要目的总结中重度急性呼吸窘迫综合征(ARDS)患者实施俯卧位通气过程中的呼吸道管理经验,规范俯卧位通气的呼吸道管理方法,保证俯卧位通气效果。方法对11例中重度ARDS患者实施俯卧位通气期间,规范进行呼吸道管理,在治疗前后对比血气分析结果,观察动脉血氧分压P(O_2)、动脉血二氧化碳分压P(CO_2)、氧合指数P(O_2)/FiO_2,监测生命体征和呼吸力学指标。结果 11例患者共实施31例次俯卧位通气,其中29例次(93.5%)俯卧位1h、4h时P(O_2)、P(O_2)/FiO_2均升高(P<0.001),P(CO_2)、气道峰压、呼吸系统顺应差异性均无统计学意义(P>0.05),血流动力学变化差异无统计学意义(P>0.05)。结论俯卧位通气过程中落实规范的呼吸道管理方法,包括充分做好翻身前人工气道评估与准备,体位转变过程中人工气道的护理和俯卧位通气期间的呼吸道管理,可有效改善中重度ARDS患者的氧合状况,达到良好效果。
