摘要
目的探讨磁共振断层血管成像(magnetic resonance tomographic angiography,MRTA)对血管压迫性脑神经病的诊断价值及对微血管减压术(microvascular decompression,MVD)的指导意义。方法收集2008年7月-2015年5月本院临床诊断并行微血管减压术的血管压迫性脑神经病患者的MRTA资料,并与MVD术结果对照分析。结果 300例中,MRTA检查发现症状侧神经血管接触(可疑接触)、压迫284例,阳性率94.7%,非症状侧神经血管接触(可疑接触)、压迫56例,阳性率18.7%,二者有统计学差异(P<0.05);300例MVD中见神经血管接触、压迫292例。以手术结果为金标准,MRTA诊断血管压迫性脑神经病的敏感性、特异性、准确性分别为99.3%、37.5%、96.0%。结论 MRTA结果与手术结果符合率高,在血管压迫性脑神经病的诊断中具有很高的价值,可清晰直观地显示血管对神经的压迫情况,是目前可靠的首选影像检查方法。
Objective To discuss the value of magnetic resonance tomographic angiography in the diagnosis of neurovascular compression syndrome and its guiding significance for microvascular decompression. Methods The MRTA data of patients with vascular compression syndrome underwent microvascular decompression from July 2008 to May 2015 were collected and compared with the results of intraoperative findings. Results Neurovascular compression or contact（dubious contacts） on symptomatic side were screened out in 284 cases with a positive rate of 94.7%, and in 56 cases for asymptomatic side with a positive rate of 18.7%, demonstrating a significant difference between the two groups（P〈0.05）. During MVD surgery, 292 cases were found with neurovascular contacts or compression. Compared with intraoperative results as the gold standard, the sensitivity, specificity and accuracy of MRTA in the diagnosis of neurovascular compression syndromes were 99.3%, 37.5%, 96.0%, respectively.Conclusion There is a high agreement between MRTA imaging and operative results. MRTA has a high dignostic accuracy in the diagnosis of neurovascular compression syndromes, and by now it is optmal and reliable imaging examination.
