膀胱尿压测评在脊髓损伤后神经源性膀胱尿潴留的规范化应用 被引量:6

摘要 目的:探讨膀胱尿压测评在脊髓损伤后神经源性膀胱尿潴留中的应用和研究,为临床管理提供量化依据。方法:将72例神经源性膀胱尿潴留患者采用随机数字表法分为试验组和对照组各36例。对照组未进行膀胱尿压测评而直接采取间歇导尿的护理方法;试验组首先进行膀胱尿压测评,在测评结果指导下进行间歇导尿。同时检测2组残余尿、排尿次数、尿细菌培养等。结果:2组从第2周起24 h排尿情况出现改善。从第4周起2组24 h平均排尿次数、平均尿量、平均漏尿次数、残余尿量、膀胱安全容量以及尿培养细菌数均比治疗前改善,差异均有统计学意义(P<0.05);从第4周起2组间24 h平均排尿次数、平均尿量、平均漏尿次数、残余尿量、膀胱安全容量以及尿培养细菌数比较,差异均有统计学意义(P<0.05)。结论:膀胱尿压测评能够为脊髓损伤后神经源性膀胱的科学管理提供量化依据,有效降低泌尿系感染,改善患者的排尿功能,从而提高生活质量。 Objective:To explore the application of bladder volume and pressure in neurogenic bladder, which would provide quan- titative basis for clinical neurogenic bladder management. Methods:Seventy-two patients with neurogenic bladder were randomly as- signed into experimental group (n=36) and control group (n=36) based on random number table. The patients in the control group were taken the current nursing management of intermittent catheterization without bladder pressure test. The patients in the experimen- tal group were taken bladder urinary pressure measurement before intermittent catheterization and conducted intermittent catheteriza- tion under the guidance of bladder urinary pressure value. Moreover, the residual urine and micturition times were recorded, and bacte- rial culture of urine was performed. Results:Both groups got improved 24 hour urination from the second week. The average number of urination, the average urine volume, the average number of leakage, residual urine volume, bladder safety capacity and urine culture bacteria significantly improved from the forth week onward compared with that before treatment (P〈0.05). The difference of the aver- age number of urination, the average urine volume, the average number of leakage, residual urine volume, bladder safety capacity and urine culture bacteria between two groups from the forth week onward were statistically significant (P〈 0.05). Conclusion:Urinary bladder pressure measurement can provide quantitative basis for the scientific management of neurogenic bladder in spinal cord injury (SCI), and can reduce urinary tract infection effectively, improve urination function and patient's life quality.
