摘要
糖尿病胃轻瘫(DGP)是在糖尿病病程中出现的以非机械性梗阻所致的胃排空延迟为主要特点的症候群,为糖尿病常见的慢性并发症之一。其主要临床症状为上腹饱胀、腹痛、早饱、恶心、呕吐等,对糖尿病患者的生活质量及血糖控制产生负面影响。其发病机制尚不明确,目前认为可能与自主神经病变、Cajal间质细胞异常、平滑肌病变、胃肠激素、高血糖等有关。近年来对DGP发病机制的相关研究显示,Cajal间质细胞受损及炎性细胞浸润等组织学变化在DGP患者中十分普遍,探讨这些组织学变化及相关调控因素的研究进展对DGP的诊断及治疗具有重要意义。
Diabetic gastroparesis （DGP） in the course of diabetes mellitus is the syndromes characterized by delayed gastric emptying due to non-mechanical obstruction. It is one of the common chronic complications of diabetes meUitus. Its main clinical symptoms include abdominal distension, abdominal pain, early satiety, anorexia, nausea, and vomiting. DGP is clearly associated with impaired quality of life, as well as erratic blood glucose control in patients with diabetes. The patho- genesis of DGP, which is currently considered to be related to autonomic neuropathy,loss of interstitial cells of Cajal, smooth muscle abnormalities, changes of gastrointestinal hormone and hyperglycaemia, has not been thoroughly clarified. Recently, the histological changes, such as loss of interstitial cells of Cajal and infiltration of inflammatory cells, have been suggested to be very common in patients with diabetic gastroparesis. Therefore, it has great significance for diagnosis and treatment of diabetic gastroparesis by discussing the research progress of histological changes and related regulatory factors.
