摘要
目的探讨医院、社区协调共管模式对60岁及以上高血压患者的防治效果。方法在社区范围内,随机抽取60岁及以上高血压患者212例,并随机分为观察组与对照组,每组106例,观察组采用医院-社区协调共管模式,对照组采用社区常规管理模式,比较两组管理、防治效果。结果观察组管理末期的规范管理率、血压控制率、服药依从性均明显高于基线值各项指标(分别为:96.23%比37.74%;83.96%比33.02%;86.79%比33.96%;均为P<0.05),也明显优于对照组(分别为:96.23%比42.45%;83.96%比39.62%;86.79%比33.02%;均为P<0.05);观察组管理末期的杜氏高血压生活质量评分显著高于基线值(P<0.01),也显著高于对照组(P<0.01)。结论医院、社区协调共管模式能明显提高社区高血压管理、防治水平,显著改善患者生活质量。
Objective To explore the prevention and treatment effects of hospital-community coordinated co-management model in hypertension patients aged 60 and over. Methods A total of 212 hypertensive patients aged 60 and over were randomly selected from the community,and randomly divided into observation group and control group,with 106 cases in each group. The observation group was managed by hospital-community coordinated co-management model,the control group was managed by the community conventional model. The effect of the management,prevention and treatment in the two groups was compared between the two groups. Results After the experiment,the standardized management rate,blood pressure control rate and medication compliance of the observation group were significantly higher than baseline values( 96. 23% vs. 37. 74%,83. 96% vs. 33. 02%,86. 79% vs. 33. 96%,respectively; all P〈0. 05),and also significantly higher than those of the control group( 96. 23% vs. 42. 45%,83. 96% vs. 39. 62%,86. 79%vs. 33. 02%,respectively; all P〈0. 05); and the Duchenne hypertension quality of life score of the observation group was significantly higher than the baseline value( P〈0. 01),which was also significantly higher than that of the control group( P〈0. 01). Conclusions The hospital-community coordinated comanagement model can significantly improve the level of the management,prevention and treatment of hypertension in the community,and the quality of life in patients with hypertension.
