摘要
急诊室病人的到达率随时间动态变化,医院现行的固定时间排班模式并不能满足病人的时变需求,因此出现病人到达高峰时段医生严重不足,病人等待时间过长,而在病人到达率低的时段医生工作时间利用率不高的现象。针对此问题,以周为单位求解医护人员的排班问题。首先基于排队论,提出一种可以估计动态需求系统中病人等待时间的解析方法,然后考虑病人和医生双方因素,在一系列规则约束下建立数学模型,以期得到最小化病人等待时间的灵活排班方案,在此基础上设计了变邻域搜索算法求解该问题。通过数值实验,验证了所提出模型和算法对医院管理工作的意义。
Patient arrival rate changes dynamically and randomly so that the current fixed shifting of physicians cannot meet the time-varying needs of patients. Therefore, on the one hand, there is a serious shortage of physicians at the peak of patient arrivals, which results in patients' long waiting time;on the other hand,there is an inefficient working time utilization of physicians at the low rate of patients' arrivals. In solving the stuff shift arrangement in hospitals on a weekly basis,first,an analytical method was proposed based on the M/M/c queuing theory to estimate the waiting time of patients in the system of dynamic demand. Second, taking both patients and physicians into consideration, a mathematical model was established under the constraint of a series of rules to obtain a flexible shifting scheme to minimize patients' waiting time, based on which a Variable Neighborhood Search(VNS)algorithm was designed to solve this problem. In the end, the significance of the proposed model and algorithm for the hospital management were verified by the numerical experiments.
