摘要
由于不能有效阻止坏死股骨头的塌陷,股骨头坏死保髋疗效如今仍然不尽如人意。根源在于传统中医宏观辨证对坏死股骨头内部的生物力学状况和塌陷相关生物力学规律普遍存在认识盲区。其疗效的获得主要是通过中药促进坏死骨组织修复来实现的,难以克服引起塌陷的生物力学因素以及塌陷后的头内生物力学不稳定状态,因此这种修复的速度和强度往往不足以有效预防坏死股骨头的塌陷与进展。
The hip-preserving curative effect of ONFH is still not satisfactory because it can not stop the collapse of necrotic femoral head. It is rooted in that the traditional Chinese medicine macroscopic syndrome differentiation has a blind area in recognizing the internal biomechanical conditions of ONFH and the collapse associated regularity of biomechanics. It works by promoting the repair of necrotic bone tissue,but the biomechanical factor which occurs collapse and the unstable state of the biological mechanics of collapsed femoral head is difficult to overcome. Therefore,the speed and intensity of the repair can not effectively prevent necrotic femoral head from collapse and advance. In view of this situation,we improve the staging system by including the indicator of microcosmic syndrome differentiation which can reflect the state of biomechanics of ONFH and the prognosis with the help of modern imaging examination method and the finite element analysis based on the micro differentiation concept. We expect to offer a theory for improve the efficacy of hip-preserving by studying the collapse pathogenesis,evaluating the collapse risk and concluding the collapse types in the early time of femoral head collapse to establish methods of diagnosis and treatment combined macroscopic and microcosmic syndrome differentiation.
