左西孟旦用于重症冠心病患者行冠脉搭桥术的围术期临床观察 被引量：3

摘要 目的:研究左西孟旦对行冠脉搭桥术的左室射血分数低下的冠心病患者围术期血流动力学的影响。方法:选取66名拟行非体外循环下冠状动脉旁路移植患者,以随机表法将其随机分为对照组(C组)和试验组(L组),两组患者在麻醉诱导后均静脉泵入多巴胺、硝酸甘油、去甲肾上腺素,L组患者诱导后静脉给予左西孟旦;C组泵入等量生理盐水,使两组患者心脏指数CI维持在2.3 L/(min·m^2)以上,选择6个不同时间点分别检测血清内心肌酶(CK-MB、cTnI)、NTproBNP的含量及患者外周血管阻力(SVR),并于术后48 h床旁超声复查患者左室射血分数(LVEF%)和术后患者使用主动脉内球囊反搏术(IABP)等情况,记录结果并进行统计学分析。结果:两组患者术前一般情况比较差异无统计学意义,L组患者术后血清心肌酶(CK-MB、cTnI)含量明显低于C组(P<0.05),且cTnI峰值有所提前(P<0.05),血清NTproBNP在T1、T3、T5,时间点明显低于C组(P<0.05),同时术后48 h床旁超声测得患者LVEF%均值,L组较C组明显提高(P<0.05)。结论:左西孟旦用于围术期左室射血分数较低的冠心病患者,可以明显改善患者预后。
