摘要
目的:探讨胎盘植入产前超声诊断的临床应用价值。方法:选择50例胎盘植入孕产妇的资料进行回顾性分析,分别采用经阴道与经腹部超声诊断。比较两组诊断结果。结果:(1)胎盘植入的超声表现主要包括如下3种:胎盘增厚、胎盘与子宫肌层界限不明显以及胎盘实质内腔隙性血流,其中占比最大的为胎盘与子宫肌层界限不明显。(2)产前超声对前壁胎盘的检出率显著高于非前壁胎盘(P<0.05),产前超声对合并中央型前置胎盘的检出率显著高于未合并中央型前置胎盘(P<0.05)。(3)合并中央型前置胎盘患者行子宫全切术与产后出血发生率均分别高于未合并中央型前置胎盘(P<0.05)。结论:胎盘植入的超声特征较多,临床诊断仍然需要进一步增强;前壁与中央型前置胎盘便于超声诊断,超声检查诊断的前置胎盘类型以及出血状况等可对患者预后状况进行预测。
Objective： To study the clinical value of prenatal ultrasound diagnosis of placenta accreta.Methods： Data of 50 cases of maternal placenta increta were retrospectively analyzed,They were perfomed transvaginal and via abdominal ultrasound diagnosis. Diagnosis results of both groups were compared.Results：（1） The sonographic findings of placenta increta mainly includes the following three kinds：placental thickening,placenta,and unobvious myometrium boundaries and interlacunar blood flow of the placenta parenchyma,and the biggest proportion was placenta and unobvious myometrium boundaries.（2）The prenatal ultrasound detection rate of anterior wall placenta was significantly higher than that of the front wall of the placenta（ P〈0.05）,the detection rate of merged the central type of placenta previa was significantly higher than that of unincorporated central type of placenta previa group（ P〈0. 05）.（3） Incidence rate of postpartum hemorrhage and hysterectomy for patients with merged central type of placenta previa were significantly higher than that of the unincorporated central type of placenta previa group（ P〈0.05）. Conclusion： More sonographic features are at present in placenta increta diagnosis,and clinical diagnosis still needs to be further enhanced. Front wall and the central type of placenta previa with the ultrasonic diagnosis show more advantageous,and ultrasound diagnosis of placenta previa and bleeding condition can be used to predict the prognosis condition of patients.
