摘要
目的:观察糖尿病肾脏病不同分期内热证分布特点及与肾脏功能相关性。方法:收集335例糖尿病肾脏病患者的中医证候及临床资料,进行统计分析。结果:(1)内热证在早中晚期广泛存在,早中期内热证出现比率最高,多与气虚证、阴虚证相结合,体现了壮火食气、热盛伤阴的发病机制。(2)与非内热证组相比,早期内热组e GFR水平更高,中晚期内热组24 h尿蛋白定量更高,晚期内热组血肌酐、尿素氮更高,二氧化碳结合力及e GFR更低。逐步回归结果显示早期e GFR与内热积分呈正相关,晚期e GFR与内热积分呈负相关,内热积分又与湿浊积分呈正相关,与气虚积分、e GFR呈负相关。结论:内热贯穿于糖尿病肾脏病病程始终,与非内热证组相比,中晚期内热可加大24 h尿蛋白排泄量,晚期内热证组血肌酐、尿素氮升高更明显,内热证组肾小球滤过率早期明显增加,晚期反而显著下降。
Objective: To investigate the distribution of Heat Syndrome and correlation with renal function in diabetic kidney disease( DKD) patients in different stages. Methods: The clinical information and TCM syndrome information of 335 DKD patients were collected and analyzed. Results:( 1) Heat Syndrome was widespread in the whole course,its widely combination with Qi and Yin Difficiency Syndrome provedthe traditional Chinese medicine theory " Superheat Consuming Qi ".( 2) Compared with non-Heat Syndrome group,The e GFR in Heat Syndrome group was higher in early-stage; the 24 h proteinuria quantification was higher in middlestage; the Scr and Bun is higher but the Eco2 and e GFR is lower in late stage. The e GFR was showed positively correlated with the Heat Syndrome Integration in early stage,but negatively correlated in late stage by Stepwise Regression Analysis. Meanwhile,the Heat Syndrome Integration had positive correlation with Dampness Syndrome Integration but negative correlation with e GFR and Qi Difficiency Syndrome Integration. Conclusion: Heat Syndrome which accentuated the renal function was widespread in the whole DKD course,it would acceleratee GFR in early-stage but deseasee GFR in late-stage.
