摘要
[目的]分析河南省南阳市张仲景医院(2012-2016)中药制剂致过敏性休克特点及规律。[方法]使用回顾性分析方法,在河南省南阳市张仲景医院病案室选择2012-2016年中药制剂致过敏性休克病历。统计专业人员采用Excel2013统计软件建立数据库,分析年龄、性别、用药情况、不良反应、给药途径、休克发生时间、转归等。[结果]纳入81例中药制剂致过敏性休克,均存在不同程度血压下降,14例(17.3%)存在过敏史,13例(16.0%)不存在过敏史,54例(66.7%)不清楚。81例中7例合并用药,均为2种。引起过敏性休克涉及中药制剂包括20多种,排在前四位的是鱼腥草、参麦注射液、双黄连注射液、清开灵注射液。过敏性休克最短发生时间在2min内,最长11d。休克患者,58例静脉给药,12例肌肉注射,9例口服,2例外用。经过抢救,77例(95.0%)痊愈,2例(2.5%)出现多器官损害,2例(2.5%)死亡。[结论]中药制剂应用中需要严格执行临床用药规律,注意患者过敏史,密切观察患者用药后的表现,及时治疗改善预后。
[Objective] The analysis of Nanyang city Zhang Zhongjing hospital（2012-2016） characteristics and regularity of anaphylactic shock induced by traditional Chinese medicine. [Method] Using retrospective analysis of medical records, 2012-2016 Nanyang city of Henan province Zhang Zhongjing hospital room selection of traditional Chinese medicine induced anaphylactic shock records. Organize the clinical data, including age, gender, medication, adverse reactions, shock clinical manifestations. Statistical professionals use Excel2013 statistical software to establish a database analysis, age, gender, medication, adverse reactions, route of administration, shock time to turn. [Results] All patients have different degree of decrease in blood pressure, 14 cases（17.3%） have a history of allergies, 13 cases（16%） there is no history of allergy, 54 cases（66.7%） is not clear in.81 cases, 7 cases with medication, were 2. Cause anaphylactic shock to traditional Chinese medicine preparation includes more than 20 kinds of, the top four is Houttuynia, Shenmai injection, Shuanghuanglian injection, Qingkailing injection, anaphylactic shock occurred in the shortest time 2min, the longest 11 d. shock patients, 58 cases of intravenous injection, intramuscular injection of 12 cases, 9 cases of oral administration, with the exception of 2. After the rescue, 77 cases（95%） cured, 2 cases（2.5%） multiple organ damage, 2 cases（2.5%） died. [Conclusion] Need to strictly enforce the law application in traditional Chinese medicine clinical medication, pay attention to patients with a history of allergies, close observation of patients after treatment, timely treatment and improve the prognosis.
