肝素结合蛋白与降钙素原在儿科重症感染中的诊断价值 被引量:4

The diagnostic value of heparin binding protein and procalcitonin in pediatric severe infection

摘要 目的探讨肝素结合蛋白(HBP)与降钙素原(PCT)在儿科重症感染中的诊断价值。方法选择2015年6月至2016年8月本院儿科收治的感染患儿为研究对象,根据感染程度分组,一般感染组40例,脓毒症组25例、严重脓毒症组11例、严重脓毒症伴休克组6例。入院后检测各组患儿HBP、PCT水平,分析两指标的临床应用价值。结果严重脓毒休克性组患儿HBP、PCT水平显著高于严重脓毒症组、脓毒症组与一般感染组,差异均有统计学意义(P<0.05);但脓毒症组与一般感染组患儿,HBP水平比较,差异无统计学意义(P=0.606)。严重脓毒症组患儿HBP、PCT水平与脓毒症组患儿分别相差10倍和3倍,严重脓毒症伴休克组患儿HBP、PCT水平与脓毒症组患儿相比,分别相差15倍和5倍。结论血浆HBP、PCT水平可作为儿科重症感染的检测指标,与PCT水平变化相比,病情越严重HBP水平升高幅度更明显,更有利于评估重症感染患者病情变化。 Objective To study the diagnostic value of heparin binding protein(HBP) and procalcitonin(PCT) in pediatric severe infection. Methods Between June 2015 and August 2016 infected children hospitalized in our hospital were selected as the research object,according to the degree of infection the patients were divided into four groups,general infection group(40 cases),25 cases in sepsis group,severe sepsis group(n= 11) and 6 cases in severe sepsis combined shock group. After admission,the levels of HBP and PCT in each group were detected,and the clinical value of the two indexes was analyzed.Results HBP and PCT levels in the severe sepsis with shock group were significantly higher than that of severe sepsis,sepsis and general infection group(P 〈0. 01). The difference of HBP level between sepsis group and general infection group was not statistically significant(P = 0.606). The levels of HBP and PCT in children with severe sepsis were 10 times and 3 times higher than those in the sepsis group,respectively. The levels of HBP and PCT in children with severe sepsis accompanied by shock were 15 times and 5 times higher than those in the sepsis group,respectively.Conclusions Plasma HBP and PCT level can be used as testing indexes of pediatric severe infection,the more severe the condition,the more pronounced the increase in HBP levels,it is more helpful for clinical assessment of patients with severe infection.
