摘要
目的:对有关老年高血压社区中医护理方面研究的疗效和安全性进行系统评价。方法:通过计算机检索Pubmed、CNKI中国知网、万方、维普数据库,搜集有关老年高血压社区中医护理方面的研究文献,检索的时间为从建库开始至2016年8月20日。进行文献的筛选和资料提取,对纳入的研究运用循证医学Rav Man5.3.5工具来进行Meta分析。结果:最终纳入11个RCT。Meta的分析结果显示:在临床显效率方面,6个RCT说明社区中医护理介入干预老年高血压临床疗效优于对照组措施常规治疗;而对于收缩压和舒张压的变化方面,4个RCT显示社区中医护理介入干预老年高血压降压效果要显著优于对照组措施常规治疗。结论:纳入研究的整体证据质量偏低,有限证据结果显示社区中医护理在治疗老年高血压的临床疗效要优于单纯常规治疗。但由于受纳入研究数量、质量较低的限制,因此需以后开展更多高质量及大样本的研究来予以验证。
Objective： Systematic review of the efficacy and safety of traditional Chinese medicine nursing in the elderly patients with hypertension. Methods： Computer searched the Pub Med, CNKI, CBM, Wan Fang and Data VIP database, collect the research literature related to TCM community nursing care for the elderly hypertension, were searched from inception to August 20, 2016. Literature selection and data extraction were carried out independently by2 researchers, and the methodological quality of the included studies was evaluated by Rav Man5.3.5 tools. Results： A total of 11 RCT. Meta analysis results show： In terms of clinical efficiency, the 6 RCT shows that the community TCM nursing intervention in elderly patients with hypertension clinical efficacy is better than the control group measures conventional treatment; For systolic and diastolic blood pressure changes, the 4 RCT shows that the community nursing intervention in elderly patients with hypertension and anti-hypertensive effect is significantly better than the control group. Conclusion： The overall quality of evidence included in the study is low.The limited evidence shows that the clinical efficacy of traditional Chinese medicine in the treatment of elderly patients with hypertension is better than that of simple routine treatment.But subject to the quantity and quality of the included studies, the conclusions may be biased, which need to be verified by more high-quality studies.
