摘要
目的观察虎驹乙型肝炎胶囊联合恩替卡韦对慢性乙型病毒性肝炎(chronic hepatitis B,CHB)患者的临床疗效及对血清炎性因子的影响.方法选取70例符合诊断及纳入标准的肝郁脾虚型CHB患者,随机分为对照组(35例)和治疗组(35例),其中对照组给予恩替卡韦片治疗,治疗组在对照组的基础上加服虎驹乙型肝炎胶囊.2组疗程均为48 wk,疗程结束后并随访24 wk.分别观察2组患者中医症候积分、病毒学应答、肝功能及血清炎性因子的水平.结果疗程结束后治疗组总有效率为91.4%,明显优于对照组(P<0.05);2组患者治疗后中医症候积分均低于治疗前,其中治疗组明显优于对照组(P<0.01);治疗组治疗结束时病毒学应答、持续病毒学应答明显优于对照组(P<0.05,P<0.01),且治疗组治疗48 wk后及随访24 wk时e抗原转阴率及e抗原转换率明显优于对照组(P<0.05);2组治疗后12、24、48 wk后丙氨酸转氨酶、天门冬氨酸氨基转移酶、总胆红素均明显降低(P<0.01),且治疗组明显优于对照组(P<0.01);治疗组治疗后肿瘤坏死因子-冄、白介素6(interleukin6,I L-6)显著降低(P<0.01),IL-10显著升高(P<0.01),且治疗组均明显优于对照组(P<0.01).结论虎驹乙型肝炎胶囊联合恩替卡韦不仅能够显著改善肝郁脾虚型CHB患者临床症状,提高乙型肝炎患者病毒应答,增加e抗原的转换,还能显著改善患者肝功能水平,调节体内炎性因子水平,疗效显著.
AIM To observe the clinical effects of Huju Yigancapsules combined with entecavir in patients with chronic hepatitis B（CHB） and the infl uence on serum infl ammatory factors.METHODS Seventy CHB patients with liver stagnation and spleen defi ciency were randomly divided into a control group（35 cases） and a treatment group（35 cases）. The control group was treated with entecavir alone, and the treatment group was given Huju Yigan capsules plus entecavir. Both groups were treated for 48 wk and then followed for 24 wk. The traditional Chinese medicine（TCM） symptom score,virological response, liver function and serum inflammatory factors were observed and compared in the two groups.RESULTS The total effective rate was was significantly higher in the observation group than in the control group（P〈0.05）. The TCM symptoms scores were lower after treatment than those before treatment in both groups, and the improvement was significantly better in the treatment group than in the control group（P〈0.01）. End-treatment virological response and sustained virological response in the treatment group were signifi cantly better than those in the control group（P〈0.05, P〈0.01）,and the e antigen negative rate and e antigen conversion rate were significantly better in the treatment group than the control group at48 wk after treatment and during the followup for 24 wk（P〈0.05）. The levels of alanine aminotransferase, aspartate aminotransferase and total bilirubin were signifi cantly reducedin both groups at 12, 24 and 48 wk（P〈0.01）,and the treatment group was significantly better than the control group（P〈0.01）. After treatment, the levels of tumor necrosis factor-αand interleukin-6（IL-6） were significantly decreased（P〈0.01）, and the level of IL-10 was signifi cantly increased（P〈0.01） in both groups,and the treatment group was signifi cantly better than the control group（P〈0.01）.CONCLUSION Huju Yigan capsules combined with entecavir can reduce TCM symptoms, increase virological response and the negative conversion of e antigen, and improve liver function and the levels of infl ammatory factors in CHB patients with liver stagnation and spleen deficiency,showing a signifi cant curative effect.
