摘要
目的比较解剖锁定板和锁骨钩钢板两种内固定术在治疗锁骨远端NeerⅡ型骨折的临床疗效。方法选取2013年9月至2015年5月收治的锁骨远端NeerⅡ型骨折患者40例,随机分成两组,A组20例采用解剖锁定板固定治疗,B组20例采用锁骨钩钢板固定治疗。记录两组患者的术中出血量、手术时间,随访并记录并发症的发生情况,应用Constant-Murley肩关节评分系统对患侧肩关节的功能进行评定。结果 A组的手术时间与B组相比无明显差异(P>0.05),而术后出血量明显少于B组,差异有统计学意义(P<0.05);A组术后并发症发生率为15%,B组为30%,差异有统计学意义(P<0.05);A组在术后3个月、术后6个月、内固定物取出1个月后及末次随访时的Constant-Murley评分均明显高于B组,差异有统计学意义(P<0.05)。结论两种方法在治疗锁骨远端NeerⅡ型骨折上均取得了较好的疗效,与锁骨钩钢板组相比,解剖锁定板组的肩关节功能较优、术中出血量和并发症较少。
Objective To compare the clinical effects of two kinds of internal fixation with anatomical locking plate and clav- icular hook plate in the treatment of distal clavicle Neer Ⅱ fractures.Methods Selected 40 cases of patients with distal clavicle Neer Ⅱ fraeturein our hospital ,treated time was from September 2013 to May 2015,randomly divided into two groups,the group A of 20 cases was treated with anatomical locking plate fixation, the group B of 20 cases was treated with clavicular hook plate fixa- tion.To record the amount of intraoperative blood loss and operation time of the two groups, and the follow-up was performed 12 to 15 months after the procedure.The incidence of complications was recorded and the function of the shoulder joint was assessed by Constant-Murley shoulder scoring system.Results There was no significant difference in the operation time between the group A and group B （P〉0.05）,and the amount of bleeding in group A was significantly less than that in group B ,and the difference was statistically significant（P〈0.05）;The incidence of postoperative complications in group A was 15%, group B was 30%, the difference was statistically significant （P〈0.05）.The Constant-Murley score of group A was significantly higher than that of group B at 3 months,6 months after operation, 1 month after fixation and at last follow-up （P〈0.05）.Conclusion Compared with the clavicular hook plate group,the anatomical locking plate group had better shoulder function,less bleeding and fewer complications.
