摘要
目的探讨MRA、TCD检查在颅内血管狭窄中的诊断对比情况。方法方便选取该院2014年6月—2016年6月接收的130例缺血性脑血管疾病患者为该次的研究对象,回顾性分析所有患者的临床资料,同时,先对患者采用TCD检查,1周内再对患者采取MRA检查,以患者的MRA检查结果为诊断标准,分析比较患者的MRA和TCD检查结果。结果该次研究的患者在经过TCD和MRA检查后,结果显示,130例患者中TCD检出动脉血管狭窄数量为78条,其中,32条大脑中动脉狭窄(MCA),14条大脑前动脉狭窄(ACA),16条大脑后动脉狭窄(PCA),颈内动脉虹吸段血管狭窄6条(CS)4条椎动脉狭窄(VA),6条基底动脉狭窄(BA)。MRA检出患者的动脉血管狭窄数为56条,其中,大脑中动脉23条,大脑前动脉5条,大脑后动脉11条,颈内动脉虹吸段出现血管狭窄6条,椎动脉5条,基底动脉6条。TCD的检查结果的特异性和敏感性略高于MRA检查的特异性与敏感性。结论 TCD和MRA对于颅内血管狭窄的灵敏性和特异性普遍较高,而TCD诊断的颅内动脉血管狭窄特异性与敏感性比MRA检测的效果更为准确一些,可以作为缺血性脑血管疾病患者常规的筛查检测方法。
Objective To study the comparison of mra and TCD in diagnosis of intracranial artery stenosis. Methods Convenient selection 130 cases of patients with ischemic cerebrovascular diseases admitted and treated in our hospital from June 2014 to June 2016 were selected and the clinical data were retrospectively analyzed, at the same time, the patients were examined by the TCD, and then MRA in 1 week, and the examination results of mra and TCD were analyzed and compared by taking the mra examination results as the diagnostic standards. Results After examination, there were 78 artery stenosis tested by TCD in 130 cases, including 32 MCA, 14 ACA, 16 PCA, 6 CS, 4 VA and 6 BA, and there were 56 artery stenosis tested by Mra, including 23 MCA, 5 ACA, 11 PCA, 6 CS, 5 VA and 6 BA, and the specificity and sensitivity of TCD examination were slightly higher than those of mra examination. Conclusion The sensitivity and specificity of TCD and mra in diagnosis of intracranial artery stenosis are high, but the sensitivity and specificity of TCD is more accurate than those of mra test, which can be used as the routine test method of patients with ischemic cerebral vascular diseases.
