摘要
目的:对比采用自体腓骨长肌腱与腘绳肌腱在全膝关节镜下移植重建前交叉韧带的临床疗效.方法:回顾性分析自2014年2月~2016年2月在解放军第一零七医院关节外科行前交叉韧带重建的患者82例,所有患者均在关节镜下进行,固定采用Rigidfix系统,将患者随机平均分为观察组(腓骨长肌腱组)40例、对照组(腘绳肌腱组)42例.分别对手术操作时间、术后第1天疼痛VAS评分及血红蛋白变化值、IKDC2000评分、Tegner评分、轴移试验进行记录和对比,对重建术后的患者按不同的阶段采取相同的恢复训练.结果:观察组、对照组均随访12 ~24个月,平均随访时间为18个月.观察组在手术操作时间、术后第1天的VAS评分及血红蛋白变化值均优于对照组,结果具有统计学意义(p<0.05),在IKDC2000评分、Tegner评分、轴移试验中,2组结果差异无统计学意义(p>0.05).结论:关节镜下采用腓骨长肌腱与腘绳肌腱重建前交叉韧带疗效确切,其中腓骨长肌腱取腱方便、操作时间短、出血量较少、术后疼痛较轻,无明显并发症.
Objective: To compare the clinical efficacy of autologous peroneus longus tendon and hamstring anterior cruciate ligament reconstruction after total knee arthroscopic transplantation. Methods: A retrospective analysis from February 2014 to February 2016 in the first 07 Hospital of PLA joint surgery underwent anterior crnciate ligament reconstruction in 82 cases, all patients were under arthroseo- py, fixed by Rigidfix system, the patients were randomly divided into A group (peroneus longus tendon group) 40 cases, group B (hamstring tendon group) 42 cases. On operation time, postoperative first VAS pain score and hemoglobin values change, IKDC2000 score, Tegner score, pivot shift test were recorded and compared for the reconstruction of postoperative patients according to the different stage to take the same recovery training. Result: A group and B group were followed up from 12 to 24 months with an average follow - up time of 18 months. A group in the operation time, postoperative day first, the VAS score and the changes of hemoglobin values were better than B group, the Result : were statistically significant ( p 〈 0.05 ), IKDC2000 score, Tegner score, pivot shift test, there was no significant difference between the two groups ( p 〉0. 05 ). Conclusion : Arthroscopic peroneus longns tendon and hamstring tendon reconstruction anterior cruciate ligament curative effect, the peroneus longus tendon were used convenient, short operation time, less bleeding, less postoperative pain, no complication.
