摘要
本文回归分析了34例左前分支传导阻滞(LAFB)患者,发现左室质量(LV_(m(?)))与(R+S)_(max)+S_m值之间呈正相关,回归方程为y=10.8+0.084x(r=0.52、P<0.01)。LV_(m(?))大于215g的27例,(R+S)_(max)+S_m为36.±11.9mm,而小于215g的7例则为25.7±4.4mm,两者差异显著(0.01<P<0.05).从回归方程求得当LV_(m(?))为215g时,(R+S)_(max)+S_m为28.86mm,取(R+S)_(max)+S_m≥29mm诊断左室肥大、敏感性、特异性、准确率及阳性预测值均较高,分别为77.8%,85.7%,79.4%及95.5%,而相同病例用S_(v1)+R_(v5)≥40mm,R_(v5)≥25mm、R_(avL)≥12mm及|S_(v1)+R_(v1)|/|R_(v5)+S_(v5)|>1诊断,则敏感性、准确率均低,后者特异性及阳性预测值也低。本文结果提示(R+SⅢ)_(max)+S≥29mm对左前分支传导阻滞患者左室肥大诊断有一定临床价值。
ECG of 34 cases with left anterior fascicular block (LAFB) was analyzed. Left ventricular(LV) mass was found to be positively correlated with value of (R+S) max+SIII. In 27 cases with LV mass more than 215g and 7 cases less than 215g, the values of (R+S) max+SIII were 36.7±11.9 and 25.7±4.4mm respectively and the difference was statistically significant. The regression equation was Y=10.8+0.084X from which (R+S) max+SIII≥29mm was deduced as a criterion for diagnosing LVH in those patients and the sensitivity, specificity, accuracy and positive predictive value were 77.8%, 85.7%, 79.4% and 95.5% respectively. It indicates that (R+S) max+SIII≥29mm is a useful criterion in the diagnosis of LVH in patients with LAFB.
