摘要
目的探讨血清白介素-10(Interleukin-10,IL-10)、转化生长因子-β1(Transforming growth factor-β1,TGF-β1)在慢性阻塞性肺疾病(Chronic obstructive pulmonary disease)炎症反应中的作用及其与FEV_1%、FEV_1/FVC及CAT评分之间的相互关系。方法同期收集慢阻肺急性加重期(AECOPD)患者、慢阻肺稳定期(SCOPD)患者、健康对照者各30例。通过酶联免疫吸附技术(ELISA)检测受试者血清IL-10和TGF-β1的水平且行肺功能(FEV_1%、FEV_1/FVC)检查、CAT评分,并对各指标进行相关性分析。结果 1.血清IL-10水平:健康组>SCOPD组>AECOPD组(P<0.01);血清TGF-β1水平:AECOPD组>SCOPD组>健康组(P<0.01);2.CAT评分:AECOPD组>SCOPD组>健康组(P<0.01);3.慢阻肺组患者血清IL-10水平与FEV_1%、FEV_1/FVC呈正相关(P均<0.01);血清TGF-β1水平与FEV_1%、FEV_1/FVC呈负相关(P均<0.01);4.慢阻肺组患者血清IL-10水平与CAT评分呈负相关(P均<0.01);血清TGF-β1水平与CAT评分呈正相关(P均<0.01);5.慢阻肺组患者CAT评分与肺功能指标FEV_1%、FEV_1/FVC呈负相关(P均<0.01)。结论 1、血清IL-10、TGF-β1参与慢阻肺的全身炎症反应且可作为判断患者病情活动度的参考指标之一,并与患者FEV_1%、FEV_1/FVC及生活质量的变化密切相关;(2)CAT评分可反映慢阻肺患者FEV_1%、FEV_1/FVC及生活质量。
Objective To explore the relationship of the serum levels of interleukin-10 and TGF-β1 with FEV_1%,FEV_1/FVC and CAT score in patients with chronic obstructive pulmonary disease. Methods It selected30 patients with AECOPD as the AECOPD group,30 patients with stable COPD as the COPD group,and 30 healthy people as the control group. Their serum levels of IL-10 and TGF-β1 were measured by ELISA. All subjects completed the CAT assessment and accepted pulmonary function test. SPSS19. 0 software package was used to analyze the statistics. Results The serum level of TGF-β1 was the highest in the AECOPD group,followed by the COPD group and the control group( P < 0. 01),and the serum level of IL-10 was the highest in the control group,followed by the COPD group and the AECOPD group( P < 0. 01). The score of CAT was the highest in the AECOPD group,followed by the COPD group and the control group( P < 0. 01). The serum level of TGF-β1 showed a negative correlation with FEV_1% and FEV_1/FVC( P < 0. 05),and the serum level of IL-10 showed a positive correlation with FEV_1% and FEV_1/FVC( P < 0. 01) in the COPD group. The serum level of TGF-β1 showed a positive correlation with the scores of CAT( P < 0. 05),and the serum level of IL-10 showed a negative correlation with the scores of CAT in the COPD group( P < 0. 01). The scores of CAT in patients with COPD were negative related to with FEV_1% and FEV_1/FVC( P < 0. 05). Conclusion IL-10 and TGF-β1 involve in the systemic inflammatory of COPD,and they can be used to assess the severity of COPD. The serum levels of IL-10 and TGF-β1 are associated with the change of FEV_1%,FEV_1/FVC and the quality of life in COPD patients. CAT is a feasible method to evaluate the quality of life and FEV_1% and FEV_1/FVC in COPD patients.
