摘要
目的分析多层螺旋CT诊断心肌桥和壁冠状动脉的临床应用价值。方法选取该院2016年4月—2017年4月期间运用多层螺旋CT行冠状动脉血管成像检查的226例患者作为研究对象,由2名CT诊断医师运用双盲法进行判定心肌桥和壁冠状动脉的存在,结果一致时诊断为心肌桥和壁冠状动脉,并对确诊患者进行壁冠状动脉长度、心肌桥厚度进行测量。结果226例患者中,发现心肌桥-壁冠状动脉41例,占比18.1%,共49处;位置分布中,前降支占比91.8%,左回旋支、右冠状动脉均占比2.0%,其他占比4.1%。41例心肌桥-壁冠状动脉患者的平均壁冠状动脉长度为(12.9±5.2)mm;平均心肌桥厚度为(1.7±1.1)mm,其中深在型9例,占比22.0%,浅在型32例,占比78.0%。结论多层螺旋CT诊断心肌桥和壁冠状动脉的临床应用价值高,属于无创检查,具有定位精准、操作简便等优势,值得推广。
Objective To analyze the clinical value of multi-slice spiral CT in the diagnosis of myocardial bridge and mural coronary artery. Methods In our hospital from April 2016 to April 2017 with 226 cases of multi-slice spiral CT coronary angiography patients as the research object, by 2 CT radiologists using double blind method was used for determination of myocardial bridge and mural coronary artery, consistent when diagnosed with heart coronary artery myocardial bridge, and diagnosis patients with coronary myocardial bridge length, wall thickness measurement. Results Of the 226 patients, there were 41 cases of myocardial bridge mural coronary artery, accounting for 18.1%, a total of 49. In the location distribution,the anterior descending branch accounted for 91.8%, the left circumflex coronary artery and the right coronary artery accounted for 2%, and the other accounted for 4.1%. The average wall length of coronary artery in 41 cases of myocardial bridge mural coronary artery was(12.9 ± 5.2) mm; the average myocardial bridge thickness was(1.7 ± 1.1) mm, among which 9 cases were deep type, accounting for 22.0%, and the superficial type was 32 cases, accounting for 78.0%. Conclusion The clinical value of multi-slice spiral CT in the diagnosis of myocardial bridge and mural coronary artery is high. It belongs to noninvasive examination, and has the advantages of accurate location and easy operation. It is worth popularizing.
