显微镜间接喉镜下低温等离子刀腺样体切除与间接喉镜下腺样体刮除术效果比较

Comparison of Effect of Microscope Indirect Laryngoscopic Plasm Radio Frequency at Low Temperature Adenoidectomy and Indirect Laryngoscopic Adenoidectomy

摘要 目的探讨显微镜间接喉镜下低温等离子刀腺样体切除与间接喉镜下腺样体刮除术的效果区别。方法 2015年1月—2017年1月之间,在该院耳鼻喉科进行腺样体肥大手术的患儿170例,按照随机数字的方法分为两个组,对照组和观察组,每组均为85例,对照组患儿利用间接喉镜下腺样体刮除术进行治疗,观察组患儿利用显微镜间接喉镜下低温等离子刀腺样体切除术进行治疗,对效果进行比较。结果对照组与观察组患儿在平均手术用时[(31.58±8.11)min vs(22.12±7.38)min],平均手术出血量[(22.36±5.34)m L vs(6.97±2.31)m L],手术后12 h VAS评分[(6.04±2.31)分vs(2.26±0.94)分]和平均住院时间[(8.94±2.54)d vs(4.12±1.48)d]4个项目的数据差异有统计学意义(t=7.954,24.387,13.910,15.116,P<0.05),从数值上看,均为对照组高于观察组,对照组共发生11例不良反应,观察组共发生2例不良反应,两组不良反应发生率的数据差异有统计学意义(χ~2=6.747,P<0.05),对照组共有8例患儿复发,观察组有1例患儿复发,两组复发数据差异有统计学意义(χ~2=5.749,P<0.05),数值上均为观察组的数据低于对照组。结论使用显微镜间接喉镜下低温等离子刀腺样体切除对腺样体肿大患儿进行治疗,手术的创伤小,对鼻咽部的组织破坏程度低,复发率明显下降,患者恢复效果良好,值得在临床范围内进行推广。 Objective To discuss the effect of microscope indirect laryngoscopic plasm radio frequency at low temperature adenoidectomy and indirect laryngoscopic adenoidectomy. Methods 170 cases of patients with adenoidal hypertrophy surgery in our hospital from January 2015 to January 2017 were selected and randomly divided into two groups with 85 cases in each, the control group and the observation group were treated with indirect laryngoscopic adenoidectomy and microscope indirect laryngoscopic plasm radio frequency at low temperature adenoidectomy, and the effect was compared between the two groups. Results The differences in the average operation time, average operation bleeding amount and VAS score in12 h after operation and average length of stay between the control group and the observation group were statistically significant, [(31.58 ±8.11)min vs(22.12 ±7.38)min,(22.36 ±5.34)mL vs(6.97 ±2.31)mL,(6.04 ±2.31)points vs(2.26 ±0.94)points,(8.94±2.54)d vs(4.12±1.48)d],(t=7.954,24.387,13.910,15.116,P〈0.05), and the values in the control group were higher than those in the observation group, in the control group, there were 11 cases with adverse reactions and in the observation group, there were 2 cases with adverse reactions and the difference in the incidence rate of adverse reactions between the two groups was statistically significant,(χ^2=6.747,P〈0.05), in the control group, 8 cases were recurrent, and in the observation group, 1 case was recurrent, and the difference in the recurrence rate between the two groups was statistically significant(χ^2=5.749,P〈0.05), and the value in the observation group was lower than that in the control group. Conclusion The operative wound of microscope indirect laryngoscopic plasm radio frequency at low temperature adenoidectomy in treatment of patients with intumescentia is small, and the destruction degree to the pharynx nasalis tissues is low and the recurrence rate is obviously decreased and the recovery effect of patients is good, which is worth clinical promotion in clinic.
