摘要
目的探讨促性腺激素释放激素类药物对真性性早熟女孩成年身高的改善效果。方法随机选取该院于2012年3月—2017年3月期间所收治的真性性早熟女孩共50例,对比经过促性腺激素释放激素类药物治疗后,身高接近成年身高的女孩其治疗开始和结束时的成年身高预测值等指标,对其影响要素进行判断。结果女孩治疗结束时的成年身高值显著优于治疗开始时的预测值,每间隔6个月增加幅度为(1.1±0.7)cm,而其治疗结束时的成年身高值、遗传靶身高以及成年身高的比较呈现为差异无统计学意义(P>0.05)。治疗结束后48例(96%)患者其FAH达到THt范围。治疗开始时,其PAH达到THt范围的患者只有20例(40%)。组间对比(χ~2=22.259 3,P<0.05)。经过促性腺激素释放激素类药物治疗后,患者的净身高为(5.1±3.9)cm(-3.2~14.3)cm,THt为(155.6±3.9)cm(147~161)cm。PAH2为(155.2±5.6)cm(146.1~166.6)cm。结论经过促性腺激素释放激素类药物治疗后,能够有效对真性性早熟女孩成年身高进行改善,同时成年身高的改变主要由治疗开始时,其女孩年龄以及预测成年身高指标有关,和遗传靶身高的损失和促性腺激素释放激素类药物治疗疗程有决定性关系。
Objective To discuss the improvement effect of gonadotropin-releasing hormone analogs in treatment of adult height of true precocious puberty girls. Methods 50 cases of true precocious puberty girls admitted and treated in our hospital from March 2012 to March 2017 were randomly selected and the adult height predictive values at the beginning and end of treatment of girls whose height was similar to that of adults were compared, and the influence factors were determined.Results At the end of treatment, the adult height value was obviously better the predictive value at the beginning of treatment, and the increasing range was（1.1±0.7）cm every six months,, at the end of treatment, there were no differences in the adult height value and genetic target height（P〉0.05）, at the end of treatment, the FAH of 48 cases（96%）reached the THt range, at the beginning of treatment, the PAH of only 20 cases（40%）reached the THt range（χ^2=22.259 3,P〈0.05）,after treatment, the stocking feet, THt and PAH2 were respectively（5.1±3.9）cm,（-3.2-14.3）cm,（155.6±3.9）cm,（147-161）cm,（155.2±5.6）cm,（146.1-166.6）cm. Conclusion The gonadotropin-releasing hormone analogs treatment can effectively improve the adult height of true precocious puberty girls, at the same time, the changes of adult height mainly start from treatment, related to the age of girls and predictive adult height index, and it also has a decisive correlation.
