摘要
目的评价在对胫骨创伤后大段骨缺损患者进行治疗的过程中,行环形外固定架骨搬运治疗的效果。方法便利选取该院于2014年3月—2015年3月期间所收治的创伤性胫骨长段骨缺损患者共20例,采用骨搬运治疗术对患者进行治疗,并对外固定的时间、骨性愈合的速度以及手术完成后肢延长的长度进行记录分析。手术完成后,患者的患侧肢体功能恢复情况则依据Paley的相关准则进行评分。结果该次入选研究的20例患者均成功进行随访,其随访时间最长为3年,最短为1年。其20例患者的骨性愈合程度良好,最长时间仅为15个月,外固定时间和患侧肢体延长长度也得到了改善。依据Paley评分,依据Paley评分,优秀13例、良好5例,尚可2例。20例患者的骨性愈合程度良好,最长时间仅为15个月,最短愈合时间为7个月,中位时间在(10.09±0.58)个月。外固定时间,最长为17个月,最短为1年,中位时间为(14.09±0.64)个月。结论骨搬运治疗方法,总体来说,操作十分便利,且对软组织的损害极小,可以较为简单的对软组织覆盖,骨缺损可以由直径一样的骨块充填和骨重建进行处理,以此在临床中得到广泛的应用。
Objective To evaluate the effect of ring external fixation bone transport treatment of massive bone defects after tibial trauma. Methods 20 cases of patients with massive bone defects after tibial trauma admitted and treated in our hospital from March 2014 to March 2015 were randomly selected and treated with bone transport surgery, and the external fixation time, bone fracture healing rate and posterior limb extended length after surgery were recorded and analyzed. Results All 20 cases selected were successfully followed up, and the longest time and shortest time of follow-up was respectively 3years and 1 year, and the fracture healing degree of 20 cases was good, and the longest time was only 15 months, and the external fixation time and affected limb extended length were improved, and the Paley score showed that 13 cases were excellent, 5 cases were good and 2 cases were fair, and the shortest time was 7 months and the median time was(10.09±0.58)months, and the longest and shortest time of external fixation was respectively 17 months and 1 year, and the median time was(14.09±0.64) months. Conclusion In conclusion, the operation of bone transportation treatment method is very simple,and the injury to the soft tissue is extremely mild, which can cover the soft tissue easily, and the bone defect can be handled by the bone filling and bone reconstruction, therefore, it is widely applied in clinic.
