摘要
目的探讨持续对口冲洗引流治疗腰椎手术后切口感染的临床效果。方法对2011年8月至2016年1月在乌海市人民医院骨科行腰椎后路内固定手术治疗后并发切口感染的13例患者采用持续对口冲洗引流治疗,对其临床资料进行回顾性分析。观察切口愈合时间、置管时间、抗生素应用时间、复发情况、不良反应等指标,评估疗效和优良率。结果 13例患者感染伤口均得到愈合,愈合时间14~19d,平均15.4d;置管时间21~36d,平均29.1d;抗生素应用7~14d,平均7.9d。临床疗效按Nakai标准:优9例、良4例,优良率为100%。随访6~18个月,无继发感染,无复发、无不良反应发生。结论采用持续对口冲洗引流治疗腰椎疾患手术后切口感染疗效满意,其伤口愈合时间短、操作简单、无需取出内固定装置、住院费用低。
Objective To investigate the clinical effect of continuous irrigation-drainage on incision infection after lumbar spine surgery.Methods Clinical data of 13 patients who underwent continuous irrigation-drainage for incision infection after posterior lumbar spine surgery in the Department of Orthopedics of Wuhai People＇s Hospital from August 2011 to January 2016 were analyzed retrospectively.The incision healing time,catheterization time,antibiotic time,recurrence rate and adverse reactions were analyzed,and the efficacy and excellent and good rate were evaluated.Results The infected wounds healed in all patients.The average incision healing time,catheterization time and antibiotic time were 15.4 days（range 14-19days）,29.1days（range 21-36 days）and 7.9 days（range 7-14days）,respectively.According to the Nakai criteria,the outcome was excellent in 9 cases and good in 4 cases.The excellent and good rate was 100%.After followup for 6-18 months,no secondary infection,recurrence and adverse reactions were found in all patients.Conclusion Continuous irrigation-drainage is satisfactory and simple for incision infection after lumbar spine surgery and results in short healing time and low hospitalization costs without the need for removal of internal fixation.
