摘要
目的总结颌面部创伤患者的处理经验。方法自2013年6月至2016年5月,对经急诊修复治疗的16 800例颌面部创伤患者,根据患者颌面部不同的伤情,应用整形外科技术最大程度恢复面部的外观形态。结果所有患者均获随访3~18个月。对皮肤裂伤者,经清创缝合后伤口无明显瘢痕增生、挛缩,面部功能及表情正常;对皮肤缺损者,治疗后无明显组织移位及牵拉变形,瘢痕不明显;对皮肤撕脱伤者,治疗后恢复了面部完整外形,瘢痕较小,畸形不明显;对合并面部骨折伤者,经及时清创、复位、缝合等治疗,无明显感染且并发症少,为后期进一步治疗打下了基础。结论将整形外科技术应用于颌面部创伤,可以获得术后瘢痕细小,后遗畸形较少的效果。
Objective To summarize the experiences of maxillofacial trauma emergency treatment. Methods From June 2013 to May 2016, the surgical procedures data of 16 800 cases with maxillofacial traumas were collected. According to the different degrees of the maxillofacial traumas, individualized surgical strategy and debridement was adopted to repair the appearance of the face to the greatest extent in accordance with plastic surgery principles. Results All patients were followed up for 3 to 18 months. For patients with skin lac- erations, no obvious scar hypertrophy or contracture occurred after debridement and relaxation suture, and the facial function and expres- sion was normal. For patients with tissue defects and avulsion, no obvious tissue displacement or deformity occurred after skin or flap trans- plantation, and the scar was not evident. Early debridement of maxillofacial fractures laid good foundations for internal fixation and follow-up care. Conclusion The application of plastic surgery principles in maxillofacial trauma reconstruction was beneficial to achieve minimal cicatrix and less sequel deformity, postoperative.
