摘要
目的探讨进展性脑梗死患者外周血内皮祖细胞的变化及其临床意义。方法选取2016年3~12月期间沧州市人民医院收治的65例进展性脑梗死患者为A组,65例完全性脑梗死患者为B组,65例健康同龄者为C组,检测并比较三组受检者的外周血内皮祖细胞,同时比较A组和B组患者中不同NIHSS评分患者的检测结果,将A组中不同时间点的检测结果也进行比较。结果 A组患者的外周血内皮祖细胞CD34、CD34/CD133、CD34/KDR及KDR分别为(13.36±1.52)%、(6.98±0.78)%、(4.82±0.45)%及(30.64±2.42)%,均低于B组的(21.52±1.99)%、(14.64±1.56)%、(9.32±0.78)%、(38.98±2.90)%及C组的(30.78±2.56)%、(20.52±1.96)%、(15.45±1.22)%、(50.45±3.62)%,B组则低于C组,差异均有统计学意义(P<0.05);A组患者中不同NIHSS评分患者的上述各项外周血内皮祖细胞检测结果均低于B组中相同NIHSS评分患者,差异均有统计学意义(P<0.05),且A组患者发病后第2、5及7天时的外周血内皮祖细胞检测结果明显低于其他时间点检测结果,差异均有统计学意义(P<0.05)。结论进展性脑梗死患者外周血内皮祖细胞呈现低表达状态,且神经缺损越严重的患者其表达水平越低,对其表达的监测有助于临床做出诊断及治疗措施的制定。
Objective To study and observe the change of peripheral blood endothelial progenitor cells(EPCs)in patients with progressive cerebral infarction, and to explore the clinical significance. Methods From March 2016 to December 2016, 65 patients with progressive cerebral infarction in our hospital were selected as group A, and 65 patients with complete cerebral infarction were selected as group B. Besides, 65 healthy persons with low risk of stroke in our hospital were selected as group C. Then the peripheral blood EPCs were detected, and then compared among the three groups, among patients with different NIHSS scores in group A and group B, and also among different time points in group A. Results The peripheral blood EPCs including CD34, CD34/CD133, CD34/KDR and KDR of group A were respectively(13.36±1.52)%,(6.98±0.78)%,(4.82±0.45)% and(30.64±2.42)%, which were all significantly lower than(21.52±1.99)%,(14.64±1.56)%,(9.32±0.78)%,(38.98±2.90)% of group B and(30.78±2.56)%,(20.52±1.96)%,(15.45±1.22)%,(50.45±3.62)% of group C, and the levels of group B were significantly lower than those of group C(P〈0.05).The detection results of patients with different NIHSS scores in group A were all significantly lower than those of patients with corresponding NIHSS scores in group B(P〈0.05), and the levels of group A on the second, fifth and seventh day after the treatment were all significantly lower than those at other time points(P〈0.05). Conclusion The patients with progressive cerebral infarction show low expression of peripheral blood EPCs, and the expression levels of patients with more severe neurological deficits are lower. The monitoring of EPCs expression is helpful for the diagnosis and treatment.
