摘要
目的对比分析3种不同植皮方式在修复大面积烧伤创面中的疗效。方法选取87例大面积烧伤患者随机分为A、B、C三组,A组患者给予邮票皮植皮,B组患者给予meek植皮,C组患者给予微粒皮植皮,比较三组患者植皮成活率、创面愈合时间、1%TBSA治疗费用、一期愈合率、死亡率及康复效果。结果 A组患者植皮成活率最高,其次是B组,C组患者植皮存活率最低,差异均具有统计学意义(P<0.05),A、B两组患者创面愈合时间差异无统计学意义(P>0.05),C组患者较A、B两组患者创面愈合时间显著延长,差异具有统计学意义(P<0.05),C组患者治疗费用最高,其次是B组,A组患者治疗费用最低,差异均具有统计学意义(P<0.05),A、B两组患者一期愈合率、死亡率差异无统计学意义(P>0.05),C组患者较A、B两组患者一期愈合率显著降低,死亡率显著升高,差异均具有统计学意义(P<0.05),三组患者关节功能康复效果差异具有统计学意义(P<0.05),B组患者康复效果最优,其次是A组患者,C组患者康复效果最差,差异均具有统计学意义(P<0.05)。结论不同植皮方式修复大面积烧伤创面的效果不经相同,在临床实际应用中要根据患者具体烧伤面积、烧伤深度面积及家庭经济状况选择合适的植皮方法。
Objective：To compare the effect of different skin grafts in repairing large area burn wounds. Methods：Eighty patients with large area bum were selected and divided into three groups： A, B and C randomly. Group A patients were given postganglionic skin grafts, group B patients were given meek skin grafts, and group C patients with micropartiele skin graft. The survival rate, wound healing time, 1% TBSA treatment cost, one healing rate, mortality rate and rehabilitation effect between the three groups were compared. Results： The survival rate of skin grafts in group A was the highest, fol- lowed by group B and group C, and the difference was significant （ P 〈 0.05 ）. There was no significant difference in wound healing time between group A and group B （ P 〉 0.05 ）. The treatment cost of group C was higher than that of group B and group B （ P 〈 0. 05 ）. The treatment cost of group C was the highest, followed by group B and group A was the lowest （ P 〈 0.05 ）. There was no significant difference in the healing rate and mortality between the two groups （ P 〉 0.05 ）. The ex- cellent and good rates of the functional rehabilitation of the three groups were 62.08%, 72.41% and 48.28% respective- ly, the difference was statistically significant （ P 〈 0.05 ）. The rehabilitation effect of group B was the best, followed by group A and group C, the difference was statistically significant （ P 〈 0.05 ）. Conclusion： The effects of different skin grafts on repairing large area bum wounds are not the same. In clinical practice, the appropriate skin graft method should be selected according to the specific bum area, bum depth area and family economic condition.
