摘要
本文报告34例有肺活检资料的艾森曼格综合征,手术治疗25例,术后早期死亡4例。肺血管病理及手术前后血液动力学变化的对比分析表明,约1/3的艾森曼格综合征患者可从手术治疗得到裨益。作者就手术适应证选择及提高外科疗效的问题提出讨论。
In 34 consecutive cases of Eisenmenger syndrome with lung biospy, 25 patients were operated with a mortality of 16%. Surgical result was mainly determined by the degree of pulmonary vascular disease(PVD).AlI of the 8 patients with grade Ⅰ to Ⅱ PVD had surgical repair without early operative death. Mean pulmonary arterial pressure(MPAP) significantly declined after operation, and decreased further one year later. In 24 patients with grade Ⅲ to Ⅳ PVD, 15 patients were operated with 4 early operative death. Residual MPAP less than 50mmHg was uncommon (2 cases), and remained severely elevated in 3 cases followed up one year later. The authors suggest that when total pulmonary resistance is greater than 1500 dyne·sec·cm~(-5), the pulmonary-systemic blood flow ratio is less than 1.2, operation is not indicated. Other clinical data should be considered in selecting borderline cases for operation. Perfect surgical repair, effective myocardial protection and good postoperative care are key aspects for excellent surgical results.
