摘要
目的探讨来曲唑联合高纯度尿促性素对多囊卵巢综合征(PCOS)患者助孕以及生殖激素变化的影响。方法选取PCOS不孕患者152例,按随机数字表法分组,对照组76例予以氯米芬联合高纯度尿促性素治疗,研究组76例予以来曲唑联合高纯度尿促性素治疗,比较2组排卵周期相关指标、卵泡发育及子宫内膜情况、血清生殖激素水平以及妊娠结局。结果研究组排卵率、单卵泡排卵率均显著高于对照组(P<0.05)。与对照组比较,研究组平均治疗周期较低,注射人绒毛膜促性腺激素(h CG)日成熟卵泡数、子宫内膜厚度较高,治疗后血清LH、FSH、E2水平显著较高,血清T水平显著较低(P<0.05)。研究组临床妊娠率显著高于对照组,自然流产率及多胎妊娠率显著低于对照组(P<0.05)。结论来曲唑联合高纯度尿促性素对PCOS患者的助孕效果较好,能够调节相关生殖激素分泌,促进卵泡发育、排出及子宫内膜生长,提高临床妊娠率,改善妊娠结局。
Objective To investigate the effects of letrozole combined with high-purity menotrophin on the pregnancy and reproductive hormones changes in patients with polycystic ovary syndrome（ PCOS）. Methods A total of 152 patients with infertility due to PCOS were selected and divided into two groups,76 cases in each group. The control group was treated with clomiphene combined with high-purity menotrophin,while the experiment group was given letrozole combined with high-purity menotrophin. Ovulation cycle related indicators,follicular development and endometrium,serum reproductive hormone levels and pregnancy outcomes of the two groups were compared. Results The ovulation rate and single follicle ovulation rate in the experiment group were significantly higher than those in the control group（ P〈0. 05）. Compared with the control group,the average treatment period was lower in the experiment group,the number of mature follicles and endometrial thickness were significantly higher after human chorionic gonadotropin（ HCG） injected,and serum LH,FSH and E2 levels were significantly higher,and serum T level was significantly lower after treatment（ P〈0. 05）. The clinical pregnancy rate in the experiment group was significantly higher than that in the control group,and the spontaneous abortion rate and multiple pregnancy rate in the experiment group were significantly lower than those in the control group（ P〈0. 05）. Conclusion The pregnancy effect of letrozole combined with high-purity menotrophin is significant in treatment of PCOS patients,it can regulate the secretion of reproductive hormones,promote follicular development,discharge and the growth of endometrium,improve the clinical pregnancy rate,and improve pregnancy outcome.
