摘要
目的:探讨精囊镜手术联合非那雄胺药物治疗慢性精囊炎患者反复血精临床有效性、安全性。方法:回顾性分析2014年1月-2016年8月精囊镜手术联合非那雄胺药物治疗36例慢性精囊炎患者反复血精的临床及随访资料,所有患者术前2周开始服用非那雄胺,应用精囊镜进行检查及治疗,术后继续应用非那雄胺4周。定期随访,观察症状及精液改善情况。结果:35例患者成功行双侧精囊镜手术,1例患者行单侧精囊镜手术,手术时间31~58 min,平均(42±9)min,术中无并发症的发生。1例失访,术后3个月随访,31例治愈(治愈率88.57%),3例好转,1例复发;术后6个月随访,30例治愈(治愈率85.71%),3例好转,2例复发;随访3、6个月时精浆果糖值与术前比较,差异均有统计学意义(P<0.05)。结论:精囊镜手术联合非那雄胺药物治疗慢性精囊炎患者反复血精安全且有效,值得临床上推广应用。
Objective：To evaluate the clinical effectiveness and safety of transurethral seminal vesiculoscopy（TUSV） combined with Finasteride in the treatment of chronic seminal vesiculitis with recurrent hemospermia.Method：From January 2014 to August 2016,the clinical and follow-up data of TUSV combined with Finasteride in the treatment of 36 patients with chronic seminal vesiculitis were retrospectively analyzed.After administration of Finasteride for 2 weeks,the patients underwent TUSV for the examination and treatment,followed by medication with Finasteride at the same dose for another 4 weeks.Postoperative follow-up was conducted for observation of the improvement of symptoms and semen.Result：TUSV was successfully accomplished in the 35 cases and one patient underwent unilateral TUSV.The operative time was 31 to 58 minutes,average（42±9）minutes.All the patients but one were followed up.After 3 months follow-up,31 cases were cured（cure rate was 88.57%）,3 cases improved,1 case of recurrence.After 6 months follow-up,30 cases were cured（cure rate was 85.71%）,3 cases improved,2 cases of recurrence.The seminal fructose value of 3,6 months follow-up were different from that of preoperative,the differences were statistically significant（P0.05）.Conclusion：Transurethral seminal vesiculoscopy combined with Finasteride is effective and safe for the treatment of chronic seminal vesiculitis with recurrent hemospermia,it is worthy of clinical application.
