持续与间断肠内营养喂养方式对危重患者肠内营养耐受性的影响分析 被引量:36

Effect of continuous and intermittent enteral feeding on critical patients' tolerance of enteral nutrition

摘要 目的 探讨危重患者采用不同肠内营养喂养方式的肠内营养耐受性.方法 按随机数字表法将80例肠内营养患者随机分成观察组和对照组,每组各40例,观察组采用间断高速喂养方式,对照组采用持续匀速喂养方式,比较两组患者的肠内营养耐受性.结果 观察组患者肠内营养耐受良好70%、耐受中等22.5%、不耐受7.5%,对照组肠内营养耐受良好72.5%、耐受中等17.5%、不耐受10%,两组比较差异无统计学意义(χ^2=0.410,P〉0.05).结论 在ACAHECⅡ评分〈20分、年龄〈60岁的危重症患者中,可优先选择间断肠内营养喂养方式;采用最新国际指南建议,每日监测肠内营养耐受性并避免不恰当终止肠内营养. Objective To investigate the effect of different enteral feeding methods on critical patients' tolerance of enteral nutrition. Methods Totally 80 patients receiving enteral nutrition feeding were equally divided into an observation group and a control group according to the random number table. Patients in the observation group received intermittent high-speed feeding,while patients in the control group received continuous feeding with constant velocity. Then the patients were compared in terms of their tolerance of enteral nutrition. Results As for patients in the observation group,70% showed good tolerance of enteral nutrition;22.5% had moderate tolerance;and the other 7.5% displayed intolerance. Among patients in the control group, 72.5% displayed good tolerance;17.5% showed moderate tolerance;and the remaining 10% had intolerance (χ2=0.410,P〉0.05). Conclusions Intermittent enteral nutrition feeding may be a priority to critical patients that score under 20 by ACAHECII and aged under 60;according to the latest international guidelines,patients' tolerance of enteral nutrition must be monitored on a daily basis,in case enteral nutrition should be stopped improperly.
