摘要
目的:系统评价逍遥散联合西药治疗抑郁症的有效性和安全性。方法:电子检索CNKI、VIP、Wan Fang、CBM、Pub Med、Cochrane图书馆等国内外数据库中逍遥散联合西药治疗抑郁症的随机对照实验(RCT),检索时间从2006年1月到2016年7月。采用Rev Man 5.3.5软件进行Meta分析。结果:共纳入26个RCT,共2239例患者。Meta分析结果显示:两组HAMD-24减分率在第6、8周[6周:OR=3.60,95%CI(1.83,7.11);8周:OR=3.76,95%CI(2.42,5.83)]时差异有统计学意义;两组HAMD-17减分率在第4周[OR=10.50,95%CI(1.14,96.58)]时差异有统计学意义,在第6、8周时差异无统计学意义。两组HAMD-24评分在第6、8周[6周:MD=-1.26,95%CI(-2.35,-0.16);8周:MD=-1.37,95%CI(-2.24,-0.49)]时差异有统计学意义;两组HAMD-17减分率在第6、8周时差异无统计学意义。14项研究报道了不良反应,两组在口干、便秘、恶心等的发生率差异有统计学意义,在头痛、震颤等的发生率差异无统计学意义。结论:逍遥散联合西药治疗抑郁症在疗效及减少不良反应上具有一定的优势,具有减毒增效的作用。受纳入文献数目和质量的影响,尚需要更多高质量的RCT予以证实。
Objective： To evaluate the clinical efficacy and safety of Xiaoyao powder combined with antidepres- sants in treatment for depression. Methods： We electronieally searched databases including the CNKI, VIP, Wan- Fang, CBM, PubMed, Cochrane Library databases to search randomized controlled trials （RCTs） of Xiaoyao powder combined with antidepressants for depression from January 2006 to July 2016. We also hand searehed relevant journals. Data was retrieved, extracted and evaluated by two reviewers independently. The Coehrane Collaboration＇s RevMan 5.3.5 software was used for data analysis. Results： The 26 RCTs involving 2220 patients were included. The results of meta-analysis showed that there was significant differences in the total effective rate at weeks 6,8 between the Xiaoyao powder combined with antidepressants and antidepressants groups （HAMD-24 score rate [six- week treatment： OR=3.60, 95%CI（1.83,7.11）, eight-week treatment： OR=3.76, 95%CI（2.42,5.83）]. The total effeetive rate （HAMD-17 score rate）[OR=10.50, 95%CI（1.14, 96.58）] at week 4 was also significant differences between the Xiaoyao powder combined with antidepressants and antidepressants groups, but there was no significant differences at weeks 6,8. As for the 24 HAMD score there was signifieant difference was noted between Xiaoyao powder combined with antidepressants and antidepressants groups at weeks 6,8 [six-week treatment：MD=-l.26, 95%Cl（- 2.35,-0.16）;eight-week treatment： MD=-1.37, 95%CI（-2.24,-0.49）]; As for the 17 HAMD score, no difference was observed between Xiaoyao powder combined with antidepressants and antidepressants groups at weeks 6,8. Side re- action： A total of 14 studies reported adverse events： there was statistically significant differences in the occurrence of dry mouth, constipation, nausea between two groups; no statistically significant difference in headache, tremor incidence. Conclusion： Therapy of Xiaoyao powder combined with antidepressants in treatment of depression was much better than antidepressants alone in total effective rate, and it also could reduce adverse effects. Due to the limited quantity and quality of ineluded studies, more high quality RCTs were needed to verify tile above conclu- siou.
