摘要
目的探讨体素内不相干运动(IVIM)理论的多b值磁共振扩散加权成像(DWI)在大鼠放疗后早期肝纤维化(HF)诊断及分期中的价值。方法将30只雌性SD大鼠随机分为3组,对右半肝分别进行1、2、3个月放疗,以制备不同病理分期的放射性大鼠肝纤维化模型。于放疗前、放疗后1、2、3个月分别对SD大鼠进行磁共振T1WI、T2WI、多b值DWI序列扫描,测量放疗前后不同时间点肝脏左右叶ADC、D、D*、f值。在放疗后的每个时间点MRI检查扫描完成后即刻处死大鼠并取病理标本。结果各时间点放疗后与放疗前对比,肝右叶ADC值与D值降低,其差别均有统计学意义(P<0.05);ADC值与D值与HF病理分期呈中度负相关;D*值及f值的差别无统计学意义(P>0.05)。结论放疗后早期HF在常规MRI序列上未见改变,ADC及D值能准确反映早期HF的严重程度,为活体评价早期HF的重要指标。
Objective Using intravoxel incoherent motion MR! (IVIM-MRI) to evaluate early stages of liver fibrosis in a radiation-induced liver fibrosis rat model. Methods dividing 30 rats into 3 groups randomly with right lobe of livers of each group irradiated for one month, two months and three months respectively to construct the fibrosis model. Each group of the rats would undergo MRI examination with the sequences of T1WI,T2 WI and multi-b values DWI before and after radiation. The mean values of ADC, D, D * and f were measured on both left and right lobes of each rats. Results The ADC and D values of the right lobe of the liver decreased significantly , which showed a moderate negative correlation with pathological stages (P 〈 0.05 ) ; but D * and f values were poorly correlated with the fibrosis level (P 〉 0.05 ). Conclusion No obvious changes of fibrosis were found on conventional MRI sequences. ADC and D values would be potential markers which could reflect the severity of early stages of hepatic fibrosis in vivo.
