摘要
目的研究对比腹会阴联合直肠癌根治术(Miles术)与保肛术应用于直肠癌患者治疗中的效果。方法 78例直肠癌患者,以随机原则分为对照组与观察组,各39例。对照组实施Miles术,予以观察组保肛术治疗。观察两组患者治疗前后生活质量及随访期间的复发情况。结果治疗前对照组与观察组生活质量量表评分分别为(51.19±4.85)分和(51.87±4.69)分,对比差异无统计学意义(P>0.05);经不同治疗后,观察组生活质量量表评分为(82.37±3.15)分,对照组为(70.92±4.60)分,两组比较,观察组明显优于对照组,差异具有统计学意义(P<0.05)。在为期6个月的随访中,对照组中有5例患者复发,复发率为12.82%;观察组中有2例患者复发,复发率为5.13%。在复发情况方面,两组比较差异无统计学意义(P>0.05)。结论保肛术应用于直肠癌患者的治疗中,效果显著,能够控制疾病发展,且无需进行肠造口操作,患者更易接受,对其生活质量的提高具有一定积极作用。
Objective To research and compare effects by abdominoperineal combined with radical resection of rectal carcinoma （Miles operation） and sphincter preserving operation in the treatment of rectal carcinoma. Methods A total of 78 patients With rectal carcinoma were randomly divided into control group and observation group, with 39 cases in each group. The control group received Miles operation, ancl the observation group received sphincter preserving operation. Observation was made on quality of life and recurrence during follow-up between the two groups. Results The control group and the observation group had quality of life score before treatment respectively as （51.19 ± 4.85） points and （51.87± 4.69） points, and their difference had no statistical significance （P〉0.05）. After treatment, the observation group had quality of life score as （82.37 ± 3.15） points, which was （70.92 ±4.60） points in the control group. The observation group had obviously better score than the control group, and the difference had statistical significance （P〈0.05）. In 6-month follow-up, there were 5 recurrent cases in the control group, with recurrence rate as 12.82%. There were 2 recurrent cases in the observation group, with recurrence rate as 5.13%. The difference of recurrence rate between the two groups had no statistical significance （P〉0.05）. Conclusion Application of sphincter preserving operation in treating patients with rectal carcinoma shows excellent effect. It can inhibit development of disease and is acceptable by patients without necessity of enterostomy. This method shows certain positive effect in improving quality of life.
