64层螺旋CT多期增强扫描对周围型肺癌与炎性假瘤的鉴别诊断 被引量:7

摘要 目的:探讨64层螺旋CT多期增强扫描对周围型肺癌与炎性假瘤的鉴别诊断作用。方法:105例确诊为周围型肺癌或炎性假瘤患者,采用64层螺旋CT行多期增强扫描,分析各期CT图像特征,寻找最有诊断价值的CT鉴别征象。结果:分叶征、长/短毛刺、切线及尖角征在周围型肺癌与炎性假瘤中的发生率不同,差异具有统计学意义(P<0.05)。周围型肺癌CT增强峰值的CT净增加值为(56.77±22.46)Hu,高于炎性假瘤增强峰值的CT净增强值(38.20±6.31)Hu(P<0.05),周围型肺癌增强扫描后120 s形成强化高峰,炎性假瘤于90 s左右出现高峰。结论:64层螺旋CT多期增强扫描能提高周围型肺癌与炎性假瘤的鉴别诊断准确率。 Objective:To investigate the differential diagnosis of peripheral lung cancer and inflammatory pseudotumor by 64 slice spiral CT multi-phase enhanced scanning.Methods:105 patients with peripheral lung cancer or inflammatory pseudotumor were treated by 64 slice spiral CT multi-phase enhanced scanning.The characteristics of each phase of CT were analyzed,and the most valuable diagnostic signs of CT were found.Results:The occurrence rate of lobulation sign,long/short burr,tangent and sharp angle sign were different in peripheral lung cancer and inflammatory pseudotumor,the differences were statistically significant(P〈0.05).The net increase of CT enhanced peak value of peripheral lung cancer was(56.77±22.46) Hu,which was higher than that of inflammatory pseudotumor(38.20±6.31) Hu(P〈0.05).Strengthening peak was observed after 120 s by enhanced scanning on peripheral lung cancer and the peak of inflammatory pseudotumor occurred at about 90 s.Conclusion:Using 64 slice spiral CT multi phase enhanced scanning can effectively improve the differential diagnostic accuracy of peripheral lung cancer and inflammatory pseudotumor.
