摘要
目的探索微波热凝在鼻内镜术后术腔病变组织的临床应用效果。方法选择慢性鼻窦炎鼻内镜术后患者60例,随机分为3组,其中治疗组20例(微波热),对照组20例(钳除法),空白组20例(生理盐水)。分别观察术后6月好转率,6个月内术腔上皮化时间,、平均换药时间,换药次数,有无再一次填塞止血。运用糖精试验法检测各组病例鼻内镜术前、术后2周、术后1个月、术后3个月的中鼻道纤毛输送速度。结果治疗组好转率为88%,对照组好转率为82%,空白组好转率为77.8%。3组比较差异有统计学意义,治疗组与对照组比较差异亦具有统计学意义(P<0.05);6个月内上皮化率,治疗组为88.9%,对照组为82.4%,空白组为77%,3组比较差异有统计学意义(P<0.05),治疗组及对照组术后2周、1个月及3个月的鼻黏膜纤毛输送速率均较空白组升高(均P<0.05),且治疗组又较对照组升高,差异均有统计学意义(P<0.05)。结论鼻内镜术后患者应用微波热凝对术腔进行处理,可以提高临床疗效及上皮化率,使治疗时间缩短,进而对患者术后恢复有利。
Objective To investigate the clinical effects of microwave coagulation therapy on patients after endoscopy surgy. Method 60 patients with chronic nasosinusitis treated in our hospital were selected to be randomly divided into 3 groups which included the treatment group （n=20, microwave）,the control group （n=20, forceps holder method） and the blank group （n=20, normal saline）.The rate of improvement, the time of epithelization within 6 months,the average time of wound clear up, the frequency of clear up and whether or not once again fillling hemostasis were observed. The nasal mucociliary transport rate was examined before operation ,at 2th weeks, 1st months and 3rd months after operation. Result The significant differences were observed in 3 groups about the rate of improvement or the rate of epithelization. The transport rate of nasal mucociliary at 2th weeks, 1st month and third months after surgury increased significantly in treatment group and control group compared to blank group, while treatment group increased than control group （P〈0.05）. Conclusion This result indicates that microwave coagulation therapy in the treatment of patients after endoscopic sinus surgery can improve the clinical curative efficacy and the rate of epithelializaion, shorten the treatment time, and it is conducive to recovery
