摘要
目的观察腹腔镜辅助阴式手术与单纯腹腔镜手术治疗子宫内膜癌的临床效果及其安全性。方法选取我院2011年1月~2012年1月收治的86例子宫内膜癌患者作为研究对象,按照手术治疗方法的不同分为对照组和观察组,各43例。对照组行单纯腹腔镜手术治疗,观察组行腹腔镜辅助阴式手术治疗,比较两组的临床效果及术后并发症发生情况。结果观察组的手术时间、术中出血量、导管留置时间、排气时间、淋巴结清除数目及住院时间等手术指标均优于对照组,差异有统计学意义(P<0.05)。观察组的术后并发症发生率为4.65%,低于对照组的16.28%,差异有统计学意义(P<0.05)。观察组的1、2、3年生存率分别为93.02%、83.72%、69.77%,均高于对照组的79.07%、65.12%、48.84%,差异有统计学意义(P<0.05)。结论腹腔镜联合阴式手术治疗子宫内膜癌的临床效果较单纯腹腔镜手术更有优势,具有手术创伤小、术后恢复快、安全性好的优点,值得临床推广使用。
Objective To observe the clinical efficacy and safety of laparoscopie assisted vaginal surgery and simple la- paroscopic surgery in the treatment of endometrial carcinoma.Methods 86 patients with endometrial carcinoma admitted in our hospital from January 2011 to January 2012 were selected as the study subjects.According to the different methods of operation,the patients were divided into the control group and the observation group,43 eases in each group.The con- trol group was treated with simple laparoscopic surgery,and the observation group was treated with laparoscopic assisted vaginal surgery.The clinical effect and the postoperative complications of the two groups were compared.Results The operation time,intraoperative blood loss,catheter retention time,exhaust time,lymph node clearance number and hospital- ization time in the observation group was better than that in the control group,with significant difference （P〈0.05）.The incidence rate of postoperative complications in the observation group was 4.65%,which was lower than that of the con- trol group （16.28%）,with significant difference （P〈O.O5）.The survival rate at 1,2,3 years of the observation group（93.02%, 83.72%69.77%） was higher than that of the control group（79.07%,65.12%,48.84%）,with significant difference （P〈0.05）. Condusion The clinical effect of laparoscopic assisted vaginal surgery in the treatment of endometrial carcinoma is superior to that of laparoscopic surgery alone.h has the advantages of small surgical trauma,quick recovery and good safety.It is worthy of clinical promotion and application.
