维生素D缺乏增加他汀类药物相关性肌病发生风险的研究进展 被引量:3

摘要 他汀类药物是目前广泛使用的心血管疾病预防和治疗的药物,是临床调脂的基础药物。肌病是其最常见的不良反应,横纹肌溶解则是其最严重的不良反应。近期发现维生素D缺乏可增加他汀类药物相关性肌病的发生风险,但这仅仅是临床观察现象,暂无机制研究的报道。本文综述了维生素D缺乏增加他汀类药物相关性肌病发生风险的相关文献,推测维生素D缺乏影响他汀类药物的可能药动学性质,即通过影响细胞色素P450酶和/或药物转运体OATP1B1的活性,从而增加他汀类药物相关性肌病发生风险。本综述期望为他汀类药物的合理使用和维生素D缺乏患者的合理用药提供科学依据和临床指导。 Statins are widely used for cardiovascular disease prevention and treatment, which is the basis of clinical lipid drugs. Myopathy is the most common adverse reaction, and rhabdomyolysis is its most serious adverse reaction. Recently reseachers found that vitamin D deficiency might increase the risk of statin myopathy, which was only clinically observed phenomena, and no mechanism was reported. This paper reviews the literatures on vitamin D deficiency increasing statin myopathy risk which might be due to vitamin D deficiency affecting the pharmacokinetic properties of stains. Vitamin D deficiency increases the risk of statin myopathy by changing cytochrome P450 enzymes and/or drug transporter OATP1B1 activity. This review expects to provide reference for the rational use of statins and guidance for patients with vitamin D deficiency.
