摘要
目的探讨应用针刺股四头肌神经入点方法对脑卒中后偏瘫患者膝关节功能障碍的干预治疗作用。方法将我院2015年2月至2016年8月收入院的脑卒中偏瘫患者70例,依照患者入院顺序随机分为对照组35例(采用康复科常规治疗);观察组35例(在对照组治疗基础上配合针刺股四头肌神经入肌点),两组患者分别治疗一个疗程(4 w)后,对照两组患者治疗前、后下肢表面肌电参数及下肢简化运动功能评分(Fugly-Meyer)做好详细记录进行数据分析,由此评价两组患者生活能力、下肢运动功能,由此推断本治疗的应用价值。结果治疗后两组患肢屈伸肌群表面肌电参数变化较治疗前明显好转,具有统计学意义;两组下肢简化评分(FMA-L)均较治疗前明显好转,具有统计学意义。结论针刺股四头肌神经入肌点能通过提高股四头肌运动单元募集的有效性,减弱拮抗运动,改善其运动控制能力及日常生活能力。
Objective To explore the application of acupuncture unit four biceps nerve entry point method in hemiplegic patients with knee joint dysfunction intervention after stroke.Methods 70 cases of stroke patients admitted in our hospital from February 2015 to August 2016,according to the admission order,were randomly divided into control group （35 cases with conventional rehabilitation therapy）,and the observation group （35 cases with acupuncture therapy in the control unit four biceps nerve entry point on the basis of the control group）.After a course of treatment （4 weeks）,comparison before and after treatment was made in terms of motor function after lower limb and lower limb EMG parameters （Fugly-Mey score of Jane Er）,by making a detailed record and data analysis and evaluating lower extremity motor function and life ability,so that the application value of the treatment could be inferred.Results Limb flexor extensor EMG parameters of the two groups improved significantly compared with before treatment,lower extremity simplified scoring （FMA-L） was better than before treatment with a significant difference.Conclusion Acupuncture unit four biceps nerve entry point can improve the motion control and daily living ability by enhancing the effectiveness of unit four biceps motor unit recruitment and weakening antagonist exercise.
