妇科手术中应用右美托咪定和丙泊酚行硬膜外麻醉的镇静效果比较

摘要 目的比较妇科手术中采用丙泊酚和右美托咪定进行硬膜外麻醉的镇静效果。方法选择2014年10月一2015年10月常德市第一中医院收治的妇科手术的患者102例,随机分为对照组和观察组各51例,对照组采用丙泊酚进行硬膜外麻醉,观察组采用右美托咪定进行硬膜外麻醉,比较两组镇静效果及不良反应等。结果两组患者心率术中下降,而术后恢复正常,与前一时间点相比差异有统计学意义(P〈0.05);而组间差异无统计学意义(P〉0.05)。两组患者术中和术后平均动脉压均低于术前,差异有统计学意义(P〈0.05);而组间差异无统计学意义(P〉0.05)。两组血氧饱和度各时间点变化不明显,组内和组问差异均无统计学意义(P〉0.05)。两组术中、术后镇静评分也均高于术前,差异有统计学意义(P〈0.05);而观察组镇静评分均高于对照组,差异有统计学意义(P〈0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P〈0.05)。结论对妇科手术患者采用右美托咪定进行硬膜外麻醉,较丙泊酚具有更佳的镇静效果,且不良发生发生率低,值得推广。 Objective To compare the sedative effect of epidural anesthesia with dexmedetomidine and propofol in gynecologic operation. Methods One hundred and two cases of patients who accepted gynecologic operation from October, 2014 to October, 2015 were selected and randomly divided into control group and observation group (with 51 cases in each group). The control group accepted propofol, while the observation group accepted dexmedetomidine. The sedative effect and adverse reactions were compared. Results The HR of two groups decreased during the operation and returned to normal after operation with statistical significance compared with the previous time point (P 〈 0.05), but the HR was similar between the two groups (P 〉 0.05). The MVP during and after operation was lower than that before operation (P 〈 0.05), while MVP of two groups was similar (P 〉 0.05). The SpO2 did not present significant changes at any time within and between the two groups (P 〉 0.05). The scores of sedative effect during and after operation were significantly higher than those before operation (P 〈 0.05), The scores of sedative effect of the observation group were significantly higher than those of control group (P 〈 0.05). The incidence of adverse reactions in observation group was lower than that in control group, and the difference was statistically significant (P 〈 0.05). Conclusion The sedative effect of dexmedetomidine is better than propofol in gynecologic operation with epidural anesthesia with less incidence of adverse reactions. Therefore, it is suitable for clinical adoption.
