摘要
目的分析初产妇无保护会阴接生法对分娩结局及会阴侧切的影响。方法选取2013～2016年厦门市第五医院收治的230例初产妇作为研究对象，随机分为观察组和对照组各115例。对照组产妇采用常规接生技术，观察组产妇采取无保护会阴接生法，分析两组患者的会阴侧切率、产后出血、会阴肿胀等情况。结果观察组会阴完整率高于对照组，差异有统计学意义（P〈0．05）；观察组会阴侧切低于对照组，差异有统计学意义（P〈0．05）。两组初产妇第二产程时间、新生）LApgar评分差异无统计学意义（P〉0．05）；观察组产时出血量少于对照组，差异有统计学意义（P〈0．05）。观察组初产妇产程疼痛程度低于对照组，差异有统计学意义（P〈0．05）。结论初产妇采用无保护会阴接生法能够降低会阴侧切率，减少产妇会阴损伤。
Objective To analyze the influence of unprotected perineum delivery on delivery outcome and lateral episiotomy. Methods Two hundred and thirty cases of primiparae admitted in the Fifth Hospital of Xiamen City from 2013 to 2016 were selected and were randomly divided into the observation group and the controf group with 115 cases in each group. The control group was treated with conventional delivery technique, while the observation group underwent unprotected perineum delivery. The analyses of lateral episiotomy rate, postpartum hemorrhage and perineal swelling and so on of two groups were conducted. Results The perineal integrity rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant （P 〈 0.05）. The lateral episiotomy rate of the observation group was significantly lower than that of the control group, and the difference was statistically significant （P 〈 0.05）. There was no significant difference in the labor time of second stage of primiparae and Apgar scores of neonates of two groups （P 〉 0,05）. The amount of bleeding of observation group during delivery was significantly less than that of control group, and the difference has statistical significance （P 〈 0.05）. The pain degree of primiparae in observation group was significantly lower than that of the control group （P 〈 0.05）. Conclusion Lateral episiotomy rate of primiparae and the maternal perineal injury can be reduced with unprotected perineum delivery.
