摘要
目的观察咪达唑仑联合芬太尼静脉复合麻醉在支气管镜检查中的安全性和舒适性。方法将拟行气管镜检查患者320例分为观察组164例和对照组156例。对照组患者给予单纯利多卡因表面麻醉,观察组患者在对照组基础上给予咪达唑仑联合芬太尼静脉复合麻醉。观察两组患者麻醉后术前、术中全程及术后经皮血氧饱和度、心率、血压的变化,比较两组患者术中频繁咳嗽、痛苦惊恐感、术后不良记忆发生率及术后患者满意度。结果两组患者术前低血压、术中缺氧、术中低血压、术后缺氧、术后心率波动及术后低血压比较,差异无统计学意义(P>0.05)。观察组麻醉后术前缺氧发生率高于对照组,而术前、术中心率波动均低于对照组(P<0.05)。观察组术中频繁咳嗽、痛苦惊恐感及术后不良记忆发生率均低于对照组,术后满意率高于对照组(P<0.05)。结论在支气管镜检查中应用咪达唑仑联合芬太尼静脉复合麻醉,是一种是舒适、安全、可靠的方法,值得临床推广。
Objective To observe the safety and the degree of comfort of intravenous combined anesthesia with midazolam and fentanyl in bronchoscopy. Methods A total of 320 patients scheduled for bronchoscopy were divided into observation group( n = 164) and control group (n = 156 ). The control group was given surface anesthesia with lidocaine alone ,while the observation group was given intravenous combined anesthesia with midazolam and fentanyl besides the anesthesia performed in the control group. The changes of transcutaneous blood oxygen saturation, heart rate and blood pressure before, during and after operation were observed in the two groups, and the incidence rates of intraoperative frequent cough, intraoperative pain and panic, and postoperative bad memory as well as the postoperative satisfaction rate were compared between the two groups. Results There was no significant difference in the incidence rate of preoperative hypotension, intraoperative hypoxia,intraoperative hypotension, postoperative hypoxia, postoperative heart rate fluctuations or postoperative hypotension between the two groups (P 〉 0.05 ). The observation group had higher incidence rate of preoperative hypoxia and lower incidence rates of preoperative and intraoperative heart rate fluctuations compared to the control group ( P 〈 0.05 ). In the observation group, the incidence rates of intraoperative frequent cough, intraoperative pain and panic, and postoperative bad memory were lower than those in the control group, and the postoperative satisfaction rate was higher than that in the control group ( P 〈 0.05 ). Conclusion Intravenous combined anesthesia with midazolam and fentanyl applied to bronchoscopy is a comfortable, safe and reliable method, which is worthy of clinical popularization.
