摘要
目的:研究和分析立体定向血肿抽吸术在基底节区出血量15~40 ml的高血压脑出血的临床治疗价值。方法:从2015年5月1日-2017年1月1日笔者所在医院收治的基底节区高血压脑出血患者中选取95例患者作为本次的研究对象,这95例患者的出血量在15~40 ml,将这95例患者分为治疗组和对照组,其中治疗组45例,对照组50例,治疗组的45例患者采用立体定向血肿抽吸术的方法进行治疗,对照组的50例患者采用不手术的保守用药治疗方式进行治疗。对比两组患者的并发症发生情况、住院时间、临床疗效,以及神经功能缺损(NIHSS)评分和日常生活质量(ADL)评分。结果:从临床治疗效果来看,治疗组患者的临床疗效显著优于对照组,差异有统计学意义(P<0.05);从神经功能缺损评分来看,治疗组患者的评分显著低于对照组,差异有统计学意义(P<0.05);从日常生活质量评分来看,治疗组患者的ADL评分明显高于对照组,差异有统计学意义(P<0.05)。结论:对于出血量在15~40 ml的基底节区高血压脑出血患者,采用立体定向血肿抽吸术进行治疗,能够显著提高临床治疗效果,降低患者的神经功能缺损评分,显著改善日常生活质量,值得在临床中推广。
Objective： To study the clinical value of stereotactic hematoma aspiration in the treatment of hypertensive intracerebral hemorrhage with basal ganglia hemorrhage in 15-40 ml.nethod： From May 1st, 2015 to January 1st, 2017, 95 patients with hypertensive intracerebral hemorrhage of basal ganglia were selected as the subjects.These 95 patients were divided into the treatment group and the control group, 45 cases in the treatment group, 50 cases in the control group, the treatment group of 45 patients used stereotactic hematoma aspiration method for treatment, the control group of 50 patients treated with conservative surgical treatment of non-surgical approach.The complications, hospital stay, clinical efficacy, neurological deficit（NIHSS） score and daily life quality（ADL） were compared between the two groups.Result： From the clinical therapeutic effect, the clinical efficacy of the treatment group was significantly better than the control group（P〈0.05）; from the neurological deficit score, the treatment group score was significantly lower than the control group（P〈0.05）, from the daily quality of life score, the treatment group of patients with ADL score was significantly higher than the control group（P〈O.O5）.Conclusion： The treatment of hypertensive intracerehral hemorrhage with basal ganglia hemorrhage in the range of 15-40 ml can improve the clinical curative effect and reduce the neurological function of the patients with stereotactic hematoma aspiration defect score, significantly improve the quality of daily life, it is worth promoting in clinical.
