摘要
目的:观察盐酸羟考酮注射液对骨科手术后患者临床镇痛效果及安全性。方法:80例骨科手术患者随机分为观察组和对照组,每组40例。患者术后均给予病人自控性静脉镇痛泵(Patient controlled intravenous analgesia,PCIA)。观察组镇痛泵配方为盐酸羟考酮注射液30 mg+0.9%氯化钠注射液稀释至100 mL;对照组为枸橼酸舒芬太尼注射液50μg+0.9%氯化钠注射液稀释至100 mL。记录患者VAS评分、PCA按压次数、MAP、HR、首次排气时间、不良反应发生情况、免疫因子及免疫功能变化。结果:观察组术后VAS评分、PCIA按压次数少于对照组,血清TNF-α以及IL-6低于对照组,且观察组的免疫细胞水平要高于对照组,差异有统计学意义(P<0.05)。观察组的不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:盐酸羟考酮注射液对骨科手术后患者的镇痛效果确切,对免疫功能的抑制较轻,不良反应少,值得临床推广应用。
Objective： To observe the clinical efficacy and safety of oxycodone hydrochloride injections for postoperative analgesia after orthopedic operations. Methods： Eighty patients undergoing elective orthopedic operations were randomly divided into observation group and control group （40 patients in each）. All patients after orthopedic operations were treated with patient controlled intravenous analgesia （ PCIA）. The patients in the observation group were given oxycodone hydrochloride injections 30 mg solvate with 0.9% normal saline to 100 mL and the patients in the control group were given sufentanil citrate injections 50 ^g diluted with 0.9% normal saline to 100 mL. After treatment, each patient was observed for VAS score, PCA pressing time, MAP, heart rate, time to first flatus and adverse reactions such as respiratory depression, nausea and vomiting, skin itching, etc, and tested for NO, substance P, inflammatory factors and immune function. Results： Compared with the control group, the VAS scores were lower and the PCA pressing times were less, and the serum levels of TNF-α and IL-6 were lower in the observation group （ P〈0.05）. The immune cell levels of the observation group were higher than the control group （ P〉0.05）. Statistical difference in the incidence of side effects between the two groups were found （P 〈0.05）.Conclusion： Oxycodone hydrochloride injections has good analgesic efficacy and less adverse reactions, can prevent the decrease of immune function, and it is worthy of clinical application.
