摘要
目的探讨个体化护理方案在胃癌营养支持患者中的应用效果。方法选取实施个体化护理模式的胃癌患者55例为观察组,选取实施常规护理模式的胃癌患者55例为对照组。术前及营养支持结束后测量患者的体重、血清白蛋白(ALB)、前白蛋白(PAB)和转铁蛋白(TRF)等营养指标及免疫指标(T细胞亚群CD4^+、CD8^+、CD4^+/CD8^+和IgG、IgA、Ig M)。实施个体化护理前后,对护理质量和患者满意度进行调查统计。结果营养支持后,两组患者的ALB、TRF和PAB水平均较本组营养支持前升高(P﹤0.05),两组间各指标水平比较,差异无统计学意义(P﹥0.05);营养支持后,两组患者的IgG、IgA、Ig M、CD4^+、CD8^+和CD4^+/CD8^+水平均较本组营养支持前升高(P﹤0.05),两组间各指标水平比较,差异无统计学意义(P﹥0.05);观察组患者对护理人员技术水平、服务态度、仪表仪容、住院环境、医护措施和巡视病情及应急处理的满意度均高于对照组患者(P﹤0.05);个体化护理方案实施后,观察组护理人员病室管理、护理书写、重患者护理、急救管理、消毒隔离和药品管理评分均较本组护理前高,差异均有统计学意义(P﹤0.05),观察组各指标评分均高于对照组,差异有统计学意义(P﹤0.05)。结论个体化护理模式在胃癌患者营养支持中的应用,主要效果体现在提高了环境整洁度,提高了护理工作效率,培养了良好的工作作风,提高了优质服务,加强和深化了安全预警意识,提高了患者对护理的满意度,值得临床推广。
Objective To investigate the effect of individualized management in nutrition support for patients with gastric carcinoma. Method 55 cases of patients with gastric carcinoma were enrolled in our study, and were adminis-tered with individualized nursing care, while another 55 cases who were given conventional nursing care were included as control. For all patients, the nutritional assessments included body weight, serum albumin （ALB）, prealbumin （PAB） and transferrin （TRF）, as well as the immune function measures （T cell subgroup CD4＋, CD8＋ and CD4＋/CD8＋, IgG, IgA, IgM）. Besides, the nursing quality and patients＆#39;satisfaction degree were investigated before and after individualized nurs-ing care. Result After nutrition support, the ALB, TRF, and PAB levels were increased in both groups （P〈0.05）, while no significant differences were observed in regard of each measure between the two groups;moreover, the IgG, IgA, IgM, CD4＋, CD8＋and CD4＋/CD8＋were also increased consistently in the two groups （P〈0.05）, and were similar between the two groups （P〉0.05）;The patients in study group had higher degree of satisfaction for technology level, service attitude, appearance, hospital environment, health care measures and nursing visits for emergencies than that of control group （P〈0.05）. After implementing the individualized nursing care, the ratings for nurses of study group in regard of ward manage-ment, nursing writing, ICU care, emergency management, disinfection and isolation and drug management scores were significantly improved （P〈0.05）, which were significantly higher than those of the control group （P〈0.05）. Conclusion Application of individualized nursing care in nutrition support for patients with gastric carcinoma may improve the envi-ronment cleanliness and work efficiency, help establish good work style and better service, reinforcing safety awareness, winning higher rating degree of satisfaction.
