摘要
背景:全髋关节置换术是治疗髋关节疾病的常用术式,可有效缓解关节疼痛症状,提高患者的生活质量,但术后很多患者会出现不同程度的平衡障碍和肌肉力量减弱,将增加跌到风险。目的:探讨单侧THA术后实施肌力平衡疗法的临床效果。方法:选取2014年1月至2015年10月行单侧THA并于术后实施肌力平衡疗法的54例患者作为肌力平衡组;同时选取2012年10月至2013年12月单侧THA术后未实施肌力平衡疗法的54例患者作为对照组,比较两组患者的临床效果。结果:两组患者的术后关节疼痛评分、关节僵硬评分、关节功能障碍评分、稳定系数、摆动幅度、跌倒指数均显著降低,下肢肌力、动态平衡显著增加。肌力平衡组患者的关节疼痛评分平均(1.2±0.8)分、关节僵硬评分平均(1.6±0.3)分、关节功能障碍评分平均(27.8±1.3)分、稳定系数13.5±0.7、摆动幅度(13.8±0.9)cm、跌倒指数(34.5±1.4)分明显低于对照组的(2.8±0.9)分、(2.8±0.4)分、(34.2±1.5)分、15.0±0.8、(15.9±1.2)cm、(40.1±2.8)分。肌力平衡组患者的下肢肌力(14.8±1.6)次、动态平衡(80.1±2.3)分、治疗总有效率为98.1%,明显高于对照组(13.0±1.5)次、(72.5±2.1)分、87.0%,均有统计学差异(P<0.05)。结论:肌力平衡疗法有助于改善单侧THA术后患者肌力平衡状况,值得临床推广使用。
Background： Total hip arthroplasty（THA） is commonly used in clinic, but the muscle strength imbalance is more common. Objective： To investigate the treatment effect of muscle strength balance in patients with unilateral THA.Methods： In this retrospective study, 54 patients undergoing unilateral THA and muscle force balance therapy between January 2014 and October 2015 were enrolled in observation group, and 54 receiving conventional unilateral THA between October 2012 and December 2013 were selected in control group. Therapeutic effect was compared between the two groups.Results： After treatment, joint pain score, joint stiffness integral, joint dysfunction score, stability coefficient, swing amplitude and fall index decreased significantly in both groups, and lower limb muscle strength and dynamic balance increased significantly. Joint pain score, joint stiffness integral, joint dysfunction score, stability coefficient, swing amplitude and fall index in observation group were significantly lower than those in control group（1.2±0.8 vs. 2.8±0.9, 1.6±0.3 vs. 2.8±0.4,27.8±1.3 vs. 34.2±1.5, 13.5±0.7 vs. 15.0±0.8, [13.8±0.9] cm vs. [15.9±1.2] cm, 34.5±1.4 vs. 40.1±2.8）. Lower limb muscle strength, dynamic balance and total effective rate in observation group were significantly higher than those in control group（14.8 ± 1.6 vs. 13.0 ± 1.5, 80.1 ± 2.3 vs. 72.5 ± 2.1, 98.1% vs. 87.0%, P〈0.05）. Conclusions： Muscle force balance therapy is helpful to improve muscle strength balance in patients with unilateral THA.
