摘要
目的探讨脑外伤患者围手术期感染的危险因素,为临床采取预防及干预措施提供依据。方法方便选择2013年1月—2015年12月神经外科收治的脑外伤患者112例,统计其围手术期内感染情况,并对危险因素进行分析。结果(1)GCS>8分、昏迷时间≤3 d、无侵入性诊疗、住院时间≤14 d的脑外伤患者围术期感染率分别为18.75%、14.29%、14.94%、12.20%。GCS≤8分、昏迷时间>3 d、住院时间>14 d、有侵入性诊疗患者的8.33%、9.38%、8.00%、9.46%,比较差异有统计学意义(P<0.05);(2)GCS、昏迷时间、住院时间、有侵入性诊疗是脑外伤围术期感染独立危险因素(OR_1=11.536,OR_2=8.445,OR_3=15.102,OR_4=7.725,P<0.05)。结论 GCS≤8分,昏迷时间、住院时间较长,有侵入性诊疗的脑外伤手术患者围术期感染风险较高,需临床给予早期预防。
Objective To discuss the infection risk factors of patients with brain trauma in perioperative period and provide basis for adopting the prevention and intervention measures in clinic. Methods 112 cases of patients with brain trauma admitted and treated in the department of neurosurgery in our hospital from January 2013 to December 2015 were convenient selected and the infection during the perioperative period was counted and the risk factors were analyzed. Results The infection rate of patients with brain trauma whose GCS ≤8 scores, coma time 〉3d, invasive diagnosis and treatment and length of stay ≤14 d was respectively 18.75%,14.29%,14.94%,12.20%, and the infection rate of patients with brain trauma whose GCS〉8 scores, coma time〉3d, length of stay 14 d, patients without the invasive diagnosis and treatment was respectively 8.33%,9.38%,8.00%,9.46%, and the differences between groups had statistical significance（P〈0.05）. The GCS, coma time, length of stay, invasive diagnosis and treatment were the independent risk factors of brain trauma during the perioperative period,（OR1=11.536,OR2=8.445,OR3=15.102,OR4=7.725,P 0.05）. Conclusion The infection risk of brain trauma patients with invasive diagnosis and treatment and longer coma time and length of stay whose GCS ≤8 scores is higher,which needs the early prevention in clinic.
