乳腺癌复发转移特点与分子分型相关性研究 被引量:2

摘要 目的探索乳腺癌术后复发转移特点与分子分型的相关性,为复发转移性乳腺癌患者进行个体化治疗提供理论依据。方法回顾性分析190例复发转移性乳腺癌患者的临床资料,统计分析复发转移特点(部位、数目、变异率、生存率)与原发灶分子分型的相关性。结果 Luminal型乳腺癌易发生骨转移(P<0.001),HER-2 enrich过表达型及Triple Negative型乳腺癌易发生内脏转移(P<0.05),同时HER-2 enrich型及Triple Negative型易存在多发转移灶,但差异无统计学意义(P>0.05)。Luminal A型变异率最高;HER-2 enrich型变异率最低,差异有统计学意义(P<0.05)。预后分析显示,各分子分型晚期乳腺癌的无进展生存期比较,差异无统计学意义(P>0.05)。结论乳腺癌的复发转移与原发灶分子分型密切相关,分子分型可作为预测患者预后的依据。 Objective To explore the correlation between the characteristics of recurrence, metastasis and molecular subtypes of breast cancer, and to provide a theoretical basis for the treatment of recurrent and metastatic of breast cancer and predict prognosis. Methods The clinical data of 190 patients with recurrent and metastatic breast cancer were retrospectively analyzed. The corre- lation between the characteristics of recurrence, metastasis (including diseased region, lesion number, aberration rate and surviv- al rate) and molecular subtypes of breast cancer was statistically analyzed. Results The breast cancer of Luminal type was prone to bone metastasis (P 〈0. 001 ) , and HER- 2 enrich and triple- negative breast cancer was prone to visceral metastasis (P 〈 0. 05). HER - 2 enrich and triple - negative breast cancer were prone to multiple metastases, but the difference was not statistically significant (P 〉 0. 05 ). The abe/ration rate of Luminal A was the highest, and the aberration rate of HER - 2 enrich was the lowest, and the difference was statistically significant (P 〈 0. 05). Prognostic analysis showed that there was no statisti- cal difference in progression - free survival among different molecular subtypes of advanced breast cancer ( P 〉 0. 05 ). Conclusion The recurrence and metastasis of breast cancer is closely related to the molecular subtypes of primary foci. Molecu- lar subtype can be used as a basis for predicting the prognosis of patients.
