摘要
目的:分析不同程度腺样体肥大与牙颌面畸形相关性。方法:纳入10~14岁不同程度腺样体肥大者60例,其中口呼吸患者15例,10~14岁无腺样体肥大患者30例,作为对照组(A组)。测量其头影侧位片A/N比值、牙颌面各项数据,按A/N比值大小分为3个组(A组:A/N<0.60;B组:0.60≤A/N<0.70;C组:A/N≥0.70,其中15例为无口呼吸者C_1组,伴口呼吸者15例为C_2组),每组30例。应用统计学分析各项项目的变异程度与腺样体肥大等级的相关性,以及单纯腺样体肥大患者与腺样体肥大伴口呼吸患者牙颌面畸形的差异。结果:A组与B组各项检测指标差异无统计学意义,C组U1NA角和LINB角大于A组(P≤0.05)。C_2组NPogSN小于及GoGnSn角大于C_1组(P≤0.05)。结论:重度腺样体肥大(A/N≥0.70)伴MB患者表现为明显的下颌后缩,下颌平面角增大和牙齿唇倾,建议上述患者于6岁前行腺样体切除术。
Objective: To study the relationship between adenoid hypotrophy and craniofacial abnormality. Methods: 60 subjects aged 10- 14 years with different degrees of adenoid hypertrophy and 30 age matched subjects without adenoid hypertrophy(the controls ) were included. The subjects were divided into three groups depending on the level of adenoid hypotrophy( Group A, the control, A/N 〈 0.60 ; Group B, 0.60≤ A/N 〈 0.70 ; Group C, A/N ≥0.70) ( n = 30,15 cases without mouth breathing in C1 group, 15 with mouth breathing in C2 group). Cephalometric measurments were obtained from the subjects, data were statistically analysed. Results: UlNA and L1NB in group C were bigger than those in group A(P≤0.05). NPogSN was smaller and GoGnSN was bigger in group C2 than those in group C1 (P ≤〈0.05 ). Conclusion: The individuals with adenoid hypotrophy (A/N ≥7.0 ) and MB present tendencies to- wards vertical craniofacial growth, convex profile and mandibular retrusion. Adenoidectomy is recommended for the children with adenoid herpertrophy before 6 years old.
